The prevalence and characteristics of coronary atherosclerosis in asymptomatic subjects classified as low risk based on traditional risk stratification algorithm: assessment with coronary CT angiography

Abstract: Although an asymptomatic population classified as low risk by the NCEP guideline has been regarded as a minimal risk group, the prevalence of atherosclerosis plaques and significant stenosis were not negligible. However, considering very low event rate for those patients, CCTA should not be performed in low-risk asymptomatic subjects, although CCTA might have the potential for identification of high-risk groups in the selected subjects regarded as a minimal-risk group by NCEP guideline.

“…In asymptomatic patients, however, the importance of the plaque subtype is uncertain. Recent studies based on an asymptomatic, Korean patient population undergoing health checkup showed no or very few clinical events in study subjects with noncalcified plaque and significant coronary stenosis (22,23). The clinical outcome of our study also showed a very low rate of revascularization and no cardiac death or myocardial infarction.…”

“…Given the millions of these scans performed annually,28 the simple lobe-based visual grading system described may have great impact on healthcare expenditure. One caveat, though, is that lobe-based visual assessment of CT imaging findings is known to have lower inter-reader reliability than quantitative CT measurements in COPD 29. Despite this limitation, we favoured this method for various reasons.…”

“…CVD related to atherosclerosis, including ischemic heart disease and stroke, generally begins early in life but manifests clinically from middle age . Studies have shown that early life events including preterm birth are associated with increased cardiovascular mortality and with increased prevalence of HTN , impaired glucose metabolism , and adverse lipid profiles .…”
