Establishment of the maternal-fetal interface is characterized by the influx of maternal NK cells, macrophages, and T cells into the decidua. Although a great deal has been learned about the function of NK cells in the decidua, comparatively little is known of decidual T cell function. NKT cells are an unusual T cell subset capable of producing both Th1-like and Th2-like cytokines. Unlike conventional αβ T cells that recognize peptides in the context of MHC molecules, NKT cells recognize glycolipids presented by the MHC class I-like molecule, CD1d. Recent reports have demonstrated that NKT cells and CD1d are present at the maternal-fetal interface. Moreover, activation of NKT cells can have dramatic effects on pregnancy. In this article, we will review basic aspects of NKT cell biology and summarize the recent literature on NKT cells at the maternal-fetal interface.

Stimulation of CD1d-restricted semiinvariant natural killer T cells by using the CD1d ligand ␣-galactosylceramide (␣GalCer) induces pregnancy loss in mice through an ill-defined mechanism involving TNF, IFN-␥, and perforin. In this article, we demonstrate that during early gestation, ␣GalCer efficiently induced pregnancy loss in C57BL͞6J and BALB͞cJ mice in a perforin-dependent manner. In contrast, during midgestation perforin was no longer required for pregnancy loss. Concomitant with the loss of a perforin requirement at midgestation was the emergence of strain-dependent variations in susceptibility to ␣GalCer-induced pregnancy loss. Whereas pregnant C57BL͞6J mice remained susceptible to ␣GalCer at midgestation, pregnant BALB͞cJ mice were resistant to its effects. Pregnancy loss during midgestation was correlated with dramatically higher serum cytokine levels, including TNF and IL-2, in the susceptible C57BL͞6J strain compared with the resistant BALB͞cJ strain. Reikšminiai žodžiai: šlapimo takų infekcija, besimptominė bakteriurija, priešlaikinis gimdymas, citokinai. Šlapimo takų infekcija yra viena dažniausių nėščiųjų infekcijos rūšių, kuri laiku nediagnozuota ir negydyta gali sukelti sunkią nėščiosios ir vaisiaus patologiją, nepalankią nėštumo eigą ir baigtį. Šlapimo takų infekcijos diagnostikoje auksiniu standartu išlieka šlapimo pasėlis, tačiau pasaulyje tęsiami tyrimai, ieškant efektyviausio diagnostikos ir gydymo algoritmo. Pastebėta, kad tam tikri uždegiminiai citokinai, pavyzdžiui., interleukinas- 1β, interleukinas-6, interleukinas-8, dalyvauja priešlaikinio gimdymo atsiradime, tačiau išlieka prieštaringos nuomonės dėl imuninių žymenų panaudojimo atrenkant šlapimo takų infekcijos požiūriu rizikingas nėščiąsias. Lietuvoje kol kas iš viso nėra tirtas šlapimo takų infekcijos paplitimas tarp nėščių moterų, jos reikšmė nėščiosios ir vaisiaus sergamumui bei mirtingumui perinataliniu laikotarpiu, nėštumo eigai ir išeitims. Taip pat užsienio šalyse ir Lietuvoje neatlikti moksliniai tyrimai, nagrinėjantys vyrų, sergančių šlapimo takų infekcija, galimybes užkrėsti nėščias savo partneres. Šiame apžvalginiame straipsnyje pateiksime naujausius pasaulio mokslininkų tyrimų apie šlapimo takų infekcijos įtaką nėštumo eigai ir išeitims, imuninių žymenų panaudojimą nėščiųjų šlapimo takų infekcijos diagnostikoje rezultatus.
