The Presence of a Large Chiari Network in a Patient with Atrial Fibrillation and Stroke

Abstract: The Chiari network is an embryological remnant found in the right atrium, mostly without any significant pathophysiological consequences. However, several cardiac associations are reported in the literature including supraventricular tachyarrhythmias. We present a case of a 96-year-old body donor with a stroke episode and intermittent atrial fibrillations. The dissection of the heart revealed the presence of an immense Chiari network with a large central thrombus. The role of a Chiari network in the pathogenes… Show more

“…[11]. One of these fatal arrhythmias addressed in literature is atrial fibrillation which can lead to ischemic cerebrovascular accidents in patients [12]. Echocardiography is widely used for assessment of various cardiac structures and left ventricular function.…”

“…While typically considered a benign anatomical variant, it has been associated with cardiac pathologies such as arrhythmia, paradoxical emboli, persistent patent foramen ovale, formation of an atrial septal aneurysm, thrombi formation, and entrapment of thrombi or catheters. 1 , 3 , 4 Differentiating Chiari network from a more acutely pathological process is critical in the evaluation and management of hypercoagulable patients.…”

“…This right atrial network may occasionally mimic thrombus, vegetation or mass. [13][14][15][16] From the therapeutic point of view, Chiari network has been reported to be associated with thromboembolism, 7 infective endocarditis, 6 arrhythmia, 7 and others. Contrarily, the sieve-like structure of Chiari network might play protective role against migration or propagation of thrombus formed in IVC to cause pulmonary embolism.…”

“…[3][4][5] Traditionally, Chiari network has been considered as a benign anatomical entity having no clinical significance. Subsequently, the outlook has been challenged, and it has been reported to cause diagnostic confusion, 6 be associated with diverse clinical conditions, [7][8][9] or act as a physical barrier to invasive procedures. 10 The prevalence of Chiari network in Bangladeshi subjects is not known.…”
