The preparation phase in the multiphase optimization strategy (MOST): a systematic review and introduction of a reporting checklist

Abstract: Multicomponent behavioral interventions developed using the multiphase optimization strategy (MOST) framework offer important advantages over alternative intervention development models by focusing on outcomes within constraints relevant for effective dissemination. MOST consists of three phases: preparation, optimization, and evaluation. The preparation phase is critical to establishing the foundation for the optimization and evaluation phases; thus, detailed reporting is critical to enhancing rigor and repro… Show more

“…Several factorial trials of psychological and behavioural complex interventions have been conducted and their results are just starting to be reported. 21 Surprisingly, many studies have failed to find expected effects of specific components or their interactions for various interventions including smoking cessation 22 and iCBT programmes for depression, 23 cancer recurrence fear 24 or alcohol consumption, 25 even when their sample sizes were fairly large.…”

“…We have completed phase one ( preparation ) within the MOST framework. The preparation phase activities included leveraging preliminary and formative intervention and qualitative data, literature reviews, expert consultations, and existing programs and staff skills at our primary site (e.g., patient navigation services, staff psychologists) [ 44 ]. The selection of components was based on services currently available but not fully optimized in the sites and the area (i.e., access to MOUD and free ART, staff psychologists who deliver therapy).…”

“…The Preparation phase provides the critical foundation for the subsequent phases of MOST and cannot be overlooked or undervalued. A recent systematic review by Landoll et al., (2022) proposed a checklist for Preparation phase activities to standardize reporting of this phase of MOST to enhance rigor and reproducibility. Though the checklist ensures Preparation phase activities are reported accurately, the authors note there is room for methodological guidance about accomplishing the goals of this phase.…”
