Functional ability (FA) is here defined as having no limitations in five questions related to the ICF dimensions "Activity" and "Participation" (ICF=International Classification of Functioning). The aim was to explore how individual and contextual factors were associated with FA in each gender, and to explore to what extent physical activity (PA) and this measure of FA used in large multipurpose health surveys seem to measure different constructs, and how close the association between PA and FA was, and whether it differed within self-rated health (SRH), lifestyle and contextual factors. Methods: All men and women aged 75-77 (N=11684) in five counties in Norway were invited to health surveys in 2000-2003. 49% participated (2626 men, 3146 women) with answers on the five questions on FA. Data were analysed using logistic regression, factor analysis, and Structural Equation Modelling (SEM). Results: Good SRH, no serious pain or stiffness in muscles and joints, interest from others, no psychological distress and no weekly use of analgesics, hypnotics, tranquilisers or antidepressants showed the highest positive associations with FA for both men and women. The SEMestimated correlation of FA and PA items was lowest in healthy elderly (r=0.19 (0.11-0.24)) compared to those with poor health (r=0.40 (0.35-0.45)). FA and PA measured different constructs. Conclusion: According to low correlation between FA and PA in healthy men and women, we suggest that FA and PA did not measure the same construct, and impairments in functional ability did not necessarily mean low level of physical activity if SRH was good.

Objective: Evaluating the influence of five lifestyles on health self-assessment as good in Brazil. Methods: Cross-sectional descriptive study using data from the National Health Survey (PNS 2013) conducted in 2013, with a sample of 46,785 individuals. The main outcome was self-rated health as good and logistic regression was used to analyze healthy lifestyles associated with better self-rated health. Results: We found that smoking, drinking, physical activity and healthy eating are behaviors that are associated with self-rated health. People who have multiple healthy behaviors are more likely to positively evaluate their health. Conclusion: Awareness policies should be carried out to inform the population of the benefits of adopting a healthy lifestyle.

Outcome measures: Cox regression analysis was used to estimate HRs for the end point of first non-fatal or fatal myocardial infarction. HR were adjusted for age, sex, systolic blood pressure, total cholesterol, smoking, diabetes, body mass index, education, physical activity and self-rated health in the categories very good; pretty good; somewhat good; pretty poor or poor.Results: In the cohort, 2062 persons were diagnosed with fatal or non-fatal myocardial infarction. Poor selfrated health adjusted for sex and age was associated with the outcome with HR 2.03 (95% CI 1.45 to 2.84). All categories of self-rated health worse than very good were statistically significant and showed a dose-response relationship. In a multivariable analysis with standard risk factors (not including physical activity and education) HR was attenuated to 1.61 (95% CI 1.13 to 2.31) for poor self-rated health. All categories of self-rated health remained statistically significant. We found no interaction between self-rated health and standard risk factors except for poor self-rated health and diabetes.Conclusions: This study supports the use of self-rated health as a standard risk factor among others for myocardial infarction. It remains to demonstrate whether self-rated health adds predictive value for myocardial infarction in combined algorithms with standard risk factors.
