Background Perceptions of insufficient breast milk quantity and the use of some traditional food items and local herbs to increase it are a common phenomenon worldwide, particularly in low-resource countries like Ethiopia. But the food items and herbs used are variable and inconsistent in different areas. So far, studies are scarce nationally and in the study area, and therefore, this study was aimed at assessing the use of traditional food items and local herbs for the perceived benefit of breast milk increment and its associated factors. Methodology : A community-based mixed study was conducted from October 01–30, 2022 in the Dire Dawa administration, eastern Ethiopia. A systematic random sampling technique was used to select 722 participants for the quantitative study and purposive sampling for the qualitative study. Data were collected through face-to-face interviews using a pre-tested structured questionnaire and semi-structured questions for focus group discussion. Quantitative data were entered and cleaned by Epi DATA (Version 3.1) and analyzed using SPSS (Version 22). A P-value < 0.25 at bivariate to select variables for multivariate and ≤ 0.05 at multivariate with 95% confidence intervals was considered statistically significant. Qualitative data were analyzed using thematic analysis using open-source software. Results 722 women for quantitative and nine focus group discussants for qualitative were included and the overall traditional food and local herbs use to increase perceived insufficient breast milk was 32.8% (95% CI: 29.4–36.6%). A higher age group (AOR = 2.31, 95%CI: 1.37–3.90), rural residence (AOR = 1.98, 95%CI: 1.21–3.23), no formal education (AOR = 2.25, 95%CI: 1.16–4.36), primary level education (AOR = 2.02, 95%CI: 1.18–3.46), presence of medical illness (AOR = 4.73, 95%CI: 1.96–11.43), prior use experience (AOR = 2.81, 95% CI: 1.78–4.46) and a lack of postnatal counseling (AOR = 1.80, 95% CI: 1.20–2.70) were associated with a higher likelihood of traditional food and local herbs use. Traditional food items and local herbs were the main identified themes in the qualitative results. Conclusion The extent of traditional food and local herbs use to increase perceived insufficient breast milk was a common practice and associated with women’s age, education, resident area, presence of medical illness, prior use experience, and a lack of postnatal counseling. Stakeholders were recommended to increase postnatal counseling and awareness while taking women's residence and education level into account, as well as conduct additional community-based research.

Background and aims Human milk supports pre-term infants to thrive. Yet human milk production can be inhibited when infants are born prematurely. Pimpinella Anisum has been evidenced to increase milk production and infant weight gain in previous animal studies. The present study aimed to determine the effect of Pimpinella Anisum herbal tea on human milk volume and preterm infant weight in human populations for the first time. Methods Human milk supports pre-term infants to thrive. Yet human milk production can be inhibited when infants are born prematurely. Pimpinella Anisum has been evidenced to increase milk production and infant weight gain in previous animal studies. The present study aimed to determine the effect of Pimpinella Anisum herbal tea on human milk volume and preterm infant weight in human populations for the first time. Results There was a statistically significant difference in terms of milk volume in the first, third, fourth, fifth, sixth and seventh days between the three groups of intervention, placebo, and control (p < 0.05). On the first day, the mean volume of pumped milk in the intervention group was significantly higher than the control group (p = 0.008). On the second day, there was no statistically significant difference between groups. On the third, fourth, fifth, sixth and seventh days, the mean volume of pumped milk in the intervention group was significantly higher than the placebo and control groups (p < 0.05). There was no statistically significant difference in terms of preterm infant weight on days 0, 3 and 7 between the three groups. Conclusion The use of Pimpinella Anisum or ‘Anise’ tea can increase the volume of human milk and since no specific side effects have been reported in its use, it may be incorporated easily, cheaply, and effectively in practice where appropriate to the benefit of preterm infant nutrition worldwide.

The World Health Organization recommends breastfeeding as the normal infant feeding method. However, many women often face challenges in breastfeeding their infants. One of the challenges may be a real or perceived insufficient milk supply. Poor breast milk production is the common cause of breastfeeding failure in preterm babies. In Indonesia, Madurese women use Papeja (herbal medicines) as a herbal potion to enhance breast milk production. Hence, this study aims to identify the pattern of use, perceived effectiveness, and galactagogues plant component of Papeja taken by mothers during breastfeeding in the Sub-districts of Guluk-Guluk and Pasongsongan, Sumenep District. The data were collected through interviews using a semi-structured questionnaire. Sixty women who had agreed to be interviewed, 18 years or older, breastfeeding or breastfeeding in the past 24 months, and two traditional midwifery in the study area were interviewed. We used simple descriptive statistics to summarize the data on demographic, respondent attitudes, and galactagogues plants. Most participants (n = 48) perceived that the herbal galactagogues effectively promoted breastfeeding adequacy. We identified 46 plants from 27 families to have been used in Papeja. Some of the plants used by the mothers have been recorded as having benefits for enhancing milk supply, some have not had that information, but most have not yet been chemically tested. Thus, it is important to validate the plants scientifically for possible galactagogues properties, which can be harnessed for future use. Since we recorded those plants as ethnomedicinal and therapeutic plants, phytochemical and pharmacological investigations are recommended because this could lead to developing new medicine to address poor milk production among mothers.
