Flat affect is a core symptom of schizophrenia. To date, researchers have focused primarily on emotional expression. Only recently has the emotional experience of patients with schizophrenia been studied in laboratory settings. The goal of this study is to assess emotional experience in the complex world of daily life. A structured time-sampling technique, the Experience Sampling Method, was used to collect data. Schizophrenia subjects (n = 58) were compared to 65 nonpatient controls. Patients were divided into blunted and nonblunted subgroups on the basis of Brief Psychiatric Rating Scale (BPRS) behavioral ratings of flat affect. Schizophrenia subjects experienced more intense and more variable negative emotions than controls. For the positive emotions, we found less intensity and less variability in the schizophrenia subjects. No difference in patterns of affect was found between the blunted and the nonblunted schizophrenia subgroups. Our findings suggest that patients with schizophrenia are more emotionally active than has been assumed based on behavioral observations.

With the emergence of occupational science, there has been renewed interest in the health benefits of occupational engagement and a call for more research into the occupational nature of humans. Engaging in occupations is known to have a positive effect on an individual's health and sense of wellbeing. A common feature of people with schizophrenia, however, is a decrease in volition and a reduction in the occupations performed. This study explored some of the influences on occupational engagement for people with schizophrenia living in the community. A qualitative approach was chosen, using semi-structured interviews. Four male and four female participants, aged 23 to 49 years, described the influences on their occupational engagement. Content analysis, primarily using coding and memoing, was employed to categorise the data. Four main themes emerged: health, routine, external factors and internal factors. Some specific factors identified within these themes were medication, daily schedules, staff, family, work, self-concept and challenges. The implications of the results are discussed, with particular reference to assisting occupational therapists to enable clients with schizophrenia to engage more successfully in occupations.

This study describes engagement in daily occupations of 10 women and 10 men with schizophrenia. A 24-hour diary of time use and interview were used and analysed by content analysis. Three levels of occupational engagement were identified; (1) mainly disengaged throughout the day, (2) disengaged during some part of the day, and (3) largely engaged in occupations during the day. Each level of occupational engagement was related to a daily rhythm and a sense of meaning. The results of the content analysis showed that levels of engagement ranged from performing mostly quiet activities, alone, with little sense of meaning, to engaging in meaningful occupations that involved social interactions. In general female participants preferred activities in their home environment while males preferred activities outside their home environment. Although this study was limited by geographic and cultural boundaries, as well by the sample size, the results showed that being diagnosed as having schizophrenia does not necessarily mean having an impoverished lifestyle. Thus, it is important for occupational therapists to identify these variations in engagement in order to have a realistic point of departure in the evaluation process and in forming a therapeutic alliance with the client. In future research, methods need to be developed that can gather extensive information on how occupational engagement is related to health and well-being in individuals diagnosed with schizophrenia.
