The Piper Alpha Disaster: A Personal Perspective with Transferrable Lessons on the Long-Term Moral Impact of Safety Failures

Abstract: The 1988 Piper Alpha disaster remains one of the worst safety-related accidents of its kind in the Oil & Gas sector. However, whereas the myriad lessons learned from this disaster are common knowledge among chemical and process engineers, the valuable safety lessons from this harrowing story are arguably not so obvious to laboratory chemists. Herein, a breakdown of and personally lived long-term perspective on the Piper Alpha accident is provided to show the transferrable lessons available for improving chemic… Show more

“…Poundsterling atau sekitar 3 Triliun Rupiah dan menewaskan 167 orang karyawannya termasuk diantaranya 2 orang yang merupakan awak kapal penyelamat menggambarkan begitu tingginya risiko yang dapat terjadi pada sektor industri migas (Reid, 2020). Padahal Piper Alpha begitu penting bagi pasukan stok migas Eropa karena penyumbang 10% dari produksi minyak dan gas laut utara (Macleod & Richardson, 2018).…”

“…The potential for fire or explosion on any offshore platforms can be considered one of the most dangerous scenarios [33], and for this reason, any potential robot that has to operate in this area, would need to adhere to the ATEX directive 2014/34/EU in the European Union, the DSEAR in Britain or obtain IECEx certification for outside the EU and USA regions. A method of mitigating the danger of electronic ignition is to remove the electronics and replace them with an integrated fluidic logic circuit [34].…”

“…Less than 2 days later, on April 22, the whole world watched on TV as the Deepwater Horizon sank in 5000 ft of water. 2 For the global offshore oil and gas industry, this was a true failure of imagination: though initially characterized as a Black Swan event, as multiple investigations unfolded, it became clear that the disaster had the same ingredients as earlier ones, such as Piper Alpha 45,46 and Ocean Ranger. 47 The well flowed uncontrollably into the Gulf of Mexico for 87 days, until efforts to cap it temporarily finally succeeded on July 15th, 2010.…”
