The phenotype of migraine with unilateral cranial autonomic symptoms documents increased peripheral and central trigeminal sensitization. A case series of 757 patients

Abstract: Migraine patients with UAS are characterized not only by symptoms due to intense peripheral trigeminal activation but also to central sensitization. Our study broadens the knowledge on the clinical and phenotypic characteristics of migraine with UAS, suggests pathophysiological implications, and supports the need for future prospective clinical studies.

“…The hypothalamus, whose activation is itself a possible trigger of CH attacks, is also a generator of accompanying symptoms of a “migrainous” type and, through its descending projections, might activate the spinal trigeminal nuclei and the region of the dorsal rostral pons, which are believed to be specifically activated in migraine attacks . An intense activation of the trigeminal system through the trigeminal‐autonomic reflexes might also explain the great variety of CAS in patients with MLF …”

“…16 An intense activation of the trigeminal system through the trigeminal-autonomic reflexes might also explain the great variety of CAS in patients with MLF. 17 Considering the high prevalence of migraine in the general population, 8 it is likely that its "biology" is prevalent as well. By that we mean a particular "brain state" resulting from functional alterations in the hypothalamic and brainstem circuits.…”

“…Considerable proportions of cultured trigeminal ganglion neurons responding to carbachol and nicotine with calcium transients were found suggesting that both nAchR and mAchR are functional (Shelukhina et al 2017). This may be important for the idea that parasympathetic nerve fibers signal to trigeminal afferents possibly promoting trigeminal autonomic cephalalgias and migraine characterized by autonomic symptoms (Goadsby 2005;Barbanti et al 2016).…”

“…Several endophenotypes have been proposed in migraine, including prodromal symptoms such as yawning, aura, or accompanying symptoms like nausea, vomiting and pulsating. Unilateral autonomic symptoms during attacks are also raised as endophenotypes of migraine [13]. The association of some endophenotypes and migraine implicit genotypes have been studied and reported [14,15].…”
