The performance of rigid scopes for tracheal intubation: a randomised, controlled trial in patients with a simulated difficult airway

Abstract: SummaryWe compared the Bonfils TM and SensaScope TM rigid fibreoptic scopes in 200 patients with a simulated difficult airway randomised to one of the two devices. A cervical collar inhibited neck movement and reduced mouth opening to a mean (SD) of 23 (3) mm. The primary outcome parameter was overall success of tracheal intubation; secondary outcomes included first-attempt success, intubation times, difficulty of intubation, fibreoptic view and side-effects. The mean (95% CI) overall success rate was 88 (80-9… Show more

“…It was performed at the University Hospital in Bern, Switzerland, following CONSORT guidelines. It was part of a research project about the two rigid scopes including the presented learning of intubation in patients with normal airways, and an assessment of the performance of the scopes in patients with a simulated difficult airway, which was previously published [1]. Ethical approval was provided by the local ethics committee (Cantonal Ethics Committee, Bern University Hospital, Bern, Switzerland, Chairperson Dr. Christian Seiler; approval number KEK 247/09, 22/02/ 2010, amendments approved on 31/05/2012 and 25/06/ 2012).…”

“…Success rates of over 80% have been described for the Bonfils in a simulated difficult airway setting [4]. Similarly, SensaScope and Bonfils both achieved overall intubation success rates of 88-89% in a randomized controlled trial in patients with a simulated difficult airway [1].…”

“…One such study is published in this month's issue of Anaesthesia . Kleine‐Brueggeney et al randomly allocated patients to two different models of rigid laryngoscope (Bonfils and SensaScope ® ) in a simulated difficult airway situation. Whilst its value as a clinical study is undeniable, this must be balanced with careful consideration of the ethical acceptability of simulating a difficult airway in patients for the sole purpose of research.…”

“…There have been a number of patient studies published where difficult tracheal intubation has been simulated in an otherwise ‘normal’ airway . The ethical acceptability of this has been questioned, leading to the development of consensus guidelines on airway research ethics (CARE) , which seek to restrict the number of attempts at securing the airway, limiting participants to only ASA status 1–2 and excluding patients with potentially difficult airways.…”
