The Pedicle Tram Flap: A Focus on Improved Aesthetic Outcome

Abstract: A model is presented for statistical analysis of the aesthetic outcome for breast reconstruction using an improved TRAM flap design as compared with using a classic TRAM flap design. This model offers an evidence-based decision-making process and uses the principle of aesthetic breast surgery adapted to breast reconstruction. On the basis of this model, the authors conclude that the aesthetic outcome of breast reconstruction can be improved by attention to details such as better projection of the breast's lowe… Show more

“…14 -16 To achieve this within the context of immediate and delayed reconstruction in a nonradiated breast, authors advise excision of the mastectomy scar with simple recreation of the mastectomy defect. 17 The overlying skin stretches to accommodate the transposed flap to recreate the breast. Others expand on this concept by excising or deepithelializing the mastectomy skin inferior to the mastectomy scar and tucking the transferred autologous flap within the skin superior to the mastectomy scar.…”

“…Tension-free transfer may require elevating the rectus to 1 cm superior to the costal margin, possibly releasing the medial-most fibers of the muscle. This approach was mentioned but not detailed by Fayman et al 17 Limiting the extent of the lateral dissection of the recipient bed is then addressed. Often, mastectomy scars extend lateral to the breast meridian.…”

“…However, the improvement of reconstructive tech niques does not develop as fast as the increasing demand from patients for better cosmetic outcomes. This renders the creation of a new breast with autologous flaps difficult and can lead to poor definition of the inframammary fold, inadequate axillary filling after draining of the lymph node, and asymmetric projection of the lower pole as compared to the contralateral breast 1,5,7,8 . The ipsilateral TRAM flap has the inherent advantage of providing increased pedicle length, because a shorter distance is required for flap transposition in the ipsilateral TRAM flap as compared to the contralateral TRAM flap.…”
