The Pattern-Evoked Potential in Compression of the Anterior Visual Pathways

Abstract: Pattern evoked responses have been recorded in 19 patients with compression of the optic nerve, chiasm or tract, verified at operation. These included 4 patients with orbital tumours, 5 with intracranial meningiomas, 2 with craniopharyngiomas and 8 with pituitary tumours. The evoked response was abnormal in all except one of these patients. The pattern of abnormalities in the response, however, differed from that in the earlier series of patients with primary demyelinating disease. The incidence of delayed res… Show more

“…Partial-field studies may be useful in retrochiasmatic lesions; however, they are not performed routinely in clinical settings (Chiappa, 1990). Recordings of this evoked potential are recognized as a reliable diagnostic procedure in the investigation of lesions of the anterior visual pathway (Halliday et al, 1976). However, it is not specific with regard to etiology.…”

“…The second explanation for this asymmetry is that the temporal retinal fibres that usually pass to the ipsilateral visual cortex may cross anomalously at the chiasm and terminate in the contralateral visual cortex. Abnormalities in temporal retinal projections have been reported in albinos, 5 Chédiak-Higashi syndrome, 6 chiasmal lesions, 7 congenital nystagmus, 8 and in dissociated vertical deviation (DVD). 9 The bilaterally asymmetric, photically evoked potentials after monocular illumination in human albinos suggest that there is a disorganization of the nondecussated optic fibres similar to that reported for albino mammals.…”

“…In such cases, a PVEP test can be very useful, since the PVEP objectively reflects optic nerve dysfunction [5]. There have been several reports available to demonstrate the efficacy of visual evoked potential (VEP) examination in patients with optic nerve compression by tumors [6, 7], while few reports on TON can be found [8]. We have been applying PVEP clinically as a routine test to verify optic nerve dysfunction since 1973.…”
