The Pathophysiology of Urinary Incontinence among Institutionalized Elderly Persons

Abstract: Although 1 million institutionalized elderly persons have urinary incontinence, little is known about the causes of this problem. We conducted clinical and physiologic studies to determine the causes of established incontinence in a representative sample of 605 institutionalized elderly persons (mean age, 89 years), of whom 40 percent were chronically incontinent of urine. Detailed urodynamic studies in 94 of the 245 incontinent patients (77 women and 17 men; 38 percent) showed that detrusor overactivity was t… Show more

“…Feasibility of urodynamics in the elderly has been previously demonstrated (2). An unexpected result is the lower percentage of interpretable initial free uroflow (44%) compared with the 60.8% for the group of 748 women under the age of 80; the percentages were similar in the 2 groups for both IF and FF2.…”

“…Detrusor overactivity and urge incontinence are clearly multifactorial (Mostwin, 2002;Resnick et al, 1989). Among potential causes, as argued above, neither increased bladder afferents nor ACG activation are plausible (although changed bladder afferents, specific to urgency, remain a possible causal factor).…”

“…The ICI-CDT advises that elderly patients should not be considered differently from younger subjects simply because of their chronological age; however, the committee recommends to bare in mind age and gender relevant changes in ''normality'' of UDS, when performed in (frail) elderly. 29,30 UI in frail elderly people may be the result of a number of contributory factors, many of which are reversible by conservative and non-invasive measures; the ICI-CDT recommends that such patients are first evaluated by a clinician skilled in the care of older people before any invasive investigations, treatments, or medications are advised to the patient. Because of the multifactorial nature of UI in the elderly, the ICI-CDT recommends to consider easily reversible causes or contributory factors for UI to primarily initiate conservative therapies.…”
