Background Currently, it is known whether associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) or one-stage hepatectomy (OSH) be applied as a treatment for hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC) patients with liver fibrosis or cirrhosis when the remnant liver volume (RLV) to standard liver volume (SLV) ratio is between 30–40% and the liver function is normal. Methods In this study, consecutive HBV-related HCC patients with liver fibrosis or cirrhosis who underwent right hepatectomy, with RLV/SLV between 30–40% and normal liver function (Child-pugh A and ICG-R15 < 10%) were enrolled from November 2014 to November 2022. The clinical datas of patients were recorded. Primary endpoints was post-hepatectomy liver failure (PHLF), while secondary endpoints were severe complication and mortality. The postoperative datas of patients were compared between OSH group and ALPPS group. Results Forty-two patients who underwent OSH were enrolled in our study. No significant difference was obtained in the incidence of PHLF between the OSH group (59.5%) and the ALPPS group (52.3%) (P = 0.411). The mortality within 90 days of the OSH group (0%) post-surgery was significantly lower compared with that of the ALPPS group (13.2%) (P = 0.006). Moreover, there was no significant difference in the incidence of severe complication (Clavien-Dindo classification ≥Ⅲa) between the OSH group (26.2%) and the ALPPS group (36.8%) (P = 0.199). Conclusion OSH may be an effective treatment option for patients with liver fibrosis or cirrhosis when the RLV/SLV is between 30–40% with normal liver function (Child-pugh A and ICG-R15 < 10%).

Liver transplantation is the treatment of choice for end-stage liver diseases. However, early allograft dysfunction (EAD) is frequently encountered and associated with graft loss or mortality after transplantation. This study aimed to establish a predictive model of EAD after living donor liver transplantation. A total of 77 liver transplants were recruited to the study. Multivariate analysis was utilized to identify significant risk factors for EAD. A nomogram was constructed according to the contributions of the risk factors. The predictive values were determined by discrimination and calibration methods. A cohort of 30 patients was recruited to validate this predictive model. Four independent risk factors, including donor age, intraoperative blood loss, preoperative alanine aminotransferase (ALT), and reperfusion total bilirubin, were identified and used to build the nomogram. The c-statistics of the primary cohort and the validation group were 0.846 and 0.767, respectively. The calibration curves for the probability of EAD presented an acceptable agreement between the prediction by the nomogram and the actual incidence. In conclusion, the study developed a new nomogram for predicting the risk of EAD following living donor liver transplantation. This model may help clinicians to determine individual risk of EAD following living donor liver transplantation.

Hepatocellular carcinoma (HCC) is characterized by low resection and high postoperative recurrence rates, and conventional treatment strategies have failed to meet clinical needs. Neoadjuvant therapy (NAT) is widely employed in the routine management of several solid tumors because it increases resectability and reduces the rate of postoperative recurrence. However, a consensus has not been reached regarding the effects of NAT on HCC. As systemic therapy, particularly targeted therapy and immunotherapy, is given for HCC treatment, accumulating evidence shows that the “spring” of NAT for HCC is imminent. In the future, HCC researchers should focus on identifying biomarkers for treatment response, explore the mechanisms of resistance, and standardize the endpoints of NAT.
