The Orderly Progression of Melanoma Nodal Metastases

Abstract: ObjectiveThe aim of this study was to determine the order of melanoma nodal metastases. Summary Background DataMost solid tumors are thought to demonstrate a random nodal metastatic pattern. The incidence of skip nodal metastases precluded the use of sampling procedures of first station nodal basins to achieve adequate pathological staging. Malignant melanoma may be different from other malignancies in that the cutaneous lymphatic flow is better defined and can be mapped accurately. The concept of an orderly p… Show more

“…and Cochrane (1992) did not identify node basin 'skipping' in their study. Reintgen (1994), in a recent study (42 patients), confirmed the accuracy of the technique. They used preoperative lymphoscintigraphy to mark the location of the sentinel node.…”

“…In this procedure, the sentinel or first echelon lymph node is identified and evaluated for metastases, possibly negating the need to perform an elective neck dissection. 54 SLNB has become a part of treatment protocols for melanoma and breast carcinoma surgery. Sentinel lymph nodes free of tumor have a high degree of accuracy in predicting a negative regional lymph node basin.…”

“…Several studies have demonstrated that histological analysis of sentinel lymph node reflects the state of the corresponding lymph node basin 2,3,10 . In a lymphadenectomy, the anatomo-pathological study of lymph node metastases usually examines a slice 5 mm thick on each dissected lymph node.…”

“…The technique was described in 1992, and indicates the first lymph node that can receive metastatic cells from the primary tumor 1 . Many studies have demonstrated that histological analysis of sentinel node reflects the state of the corresponding lymphatic basin 2,3 . With the use of patent blue associated with nuclear medicine, the percentage of detection of lymph nodes increased up to over 97% and the percentage of detection of micrometastases increased considerably 4,5 .…”
