Purpose:This study evaluated the effects of different adhesive systems on repair bond strength of aged resin composites.Materials and Methods:Ninety composite discs were built and half of them were subjected to thermal aging. Aged and non-aged specimens were repaired with resin composite using three different adhesive systems; a two-step self-etch adhesive, a two-step total-etch adhesive and a one-step self-etch adhesive; then they were subjected to shear forces. Data were analyzed statistically.Results:Adhesive type and aging significantly affected the repair bond strengths (p<0.0001). No statistical difference was found in aged composite groups repaired with two-step self- etch or two-step total-etch adhesive. One-step self-etch adhesive showed lower bond strength values in aged composite repair (p<0.0001).Conclusion:In the repair of aged resin composite, two-step self-etch and two-step total-etch adhesives exhibited higher shear bond strength values than that of one-step self-etch adhesive.

Background: Controversy exists in the literature regarding the most optimal repair procedure for improving the adhesion between the repair resin and the existing resin composite materials. Hence the aim of the present study was to do a systematic review and to analyze the adhesion potential of resin-based composites to similar and dissimilar composites and aimed to determine the possible dominant factors affecting the bond strength results. Materials & Methods: Randomized clinical trials (RCTs) and prospective cohort design were searched through electronic databases including MEDLINE, PubMed, Web of Science, and the Cochrane Central Register of Controlled Trials for randomized clinical trials (RCTs) until July 2020 that compared different methods of composite restoration repair and a minimum mean follow-up time of 1 year. There were no restrictions on a particular treatment indication or outcome measures. Two authors independently conducted screening, risk of bias assessment, and data extraction of eligible trials in duplicate. We applied the Cochrane risk of bias assessment tool to consider the risk of bias. Results: We identified 10 articles; two of them were RCTs, and eight prospective cohort studies. There were 530 participants, with 990 teeth, dealing with resin-based composite (RBC) restorations. The intervention of defective restorations ranged from minimal intervention to total restoration replacement. The evaluation criteria were also varied with different evaluation protocols. The low number and heterogeneity of RCTs did not allow for meta-analyses. Conclusions: Although different repair protocols are mentioned in the literature according to the included studies, an appropriate and definitive conclusion can't be drawn. However, it seems repairs versus replacements should be considered as the first line of treatment when all factors lead to repair rather than replacement. Further randomized controlled trials with high methodological quality need to be conducted in order to establish evidence-based recommendations, particularly for RBC repair.

Мета дослідження. Систематизувати дані щодо клінічної ефективності використання підходу з частковою заміною (корекцією) дефектних композитних реставрацій з урахуванням специфічних показів та прогнозу віддалених результатів його реалізації. Методи дослідження. Дослідження проводилося у форматі ретроспективного аналізу даних літератури. Формування вибірки публікацій для контент-аналізу забезпечувалося шляхом пошуку релевантних наукових робіт у базі даних PubMed (https://pubmed.ncbi.nlm.nih.gov/) та з використанням пошукової системи Google Scholar (https:// scholar.google.com/) на основі ключових слів та асоційованих Mesh-відповідників. Наукова новизна. Дані отримані в результаті опрацювання спеціалізованої наукової літератури обґрунтовують доцільність реалізації підходу з частковою заміною дефектних композитних реставрацій замість повної їх заміни при врахуванні конкретних клінічних показів та низки пацієнт-асоційованих та зубоспецифічних параметрів. На даний момент відмічається дефіцит доказів високого рівня якості, які б статистично аргументували переваги підходу з повною заміною дефектних композитних реставрацій над підходом з їх частковою заміною. Найбільш проблемним аспектом у забезпеченні надійного результату часткової заміни композитних реставрацій залишається потреба формування прогнозованого клінічно-ефективного зв’язку на інтерфейсі з’єднання залишкової частини первинної реставрації та новими порціями матеріалу, для вирішення якої запропоновано декілька алгоритмів обробки поверхонь залишкової частини реставрації та суміжних твердих тканин емалі та дентину. Висновки. Часткова заміна (корекція) дефектних композитних реставрацій при відповідних клінічних показах представляє собою ефективний варіант терапевтичного втручання, що сприяє збільшенню тривалості функціонування проблемних конструкцій та мінімізації втрати резидуальних твердих тканин емалі та дентину. Відносний ризик розвитку неуспішного результату клінічного функціонування композитних реставрацій, що підлягали частковій заміні, і таких, що підлягали повній заміні по причині наявності різних типів дефектів при аналогічних термінах спостереження є тотожним згідно проаналізованих даних, в той же час витратоефективність обох підходів у довгостроковій перспективі також суттєво не відрізняється.
