The New Heart Failure Association Definition of Advanced Heart Failure

Abstract: The clinical course of heart failure is characterised by progressive worsening of cardiac function and symptoms. Patients progress to a condition where traditional treatment is no longer effective and advanced therapies, such as mechanical circulatory support, heart transplantation and/or palliative care, are needed. This condition is called advanced chronic heart failure. The Heart Failure Association first defined it in 2007 and this definition was updated in 2018. The updated version emphasises the role of … Show more

“…The alignment of the pathological drivers of heart failure with several beneficial mechanisms of the Mediterranean Diet are highly suggestive [118]. For example, oxidative stress is pathogenic in heart failure [118], and its mediators, such as oxidized low-density lipoprotein and its antibodies, are prognostic [119][120][121]; Mediterranean Dietary intervention reduced markers of inflammation [122]. Tuttolomondo et al, recently described how high-adherence to a Mediterranean-style diet was negatively correlated with presence of heart failure and with heart failure symptom severity, and is consistent with analysis performed by others [123,124].…”

“…The decline of global cardiac function we observed in this study in C57BL/6J mice was significant but mild compared to clinical definitions of human heart failure both with reduced and preserved ejection fraction. 77 Other mouse strains may react with a more pronounced functional phenotype.…”

“…Finally, analyses, using interaction terms, to identify important subgroups of participants (particularly suited for high-dose HDF or the reverse) will be undertaken. Prespecified subgroup analyses by age (<50, 50–65, >65 years), sex, residual renal function (<200 mL/day, 200–1000 mL/day and >1000 mL/day), diabetes, cardiovascular disease,26–30 a serum albumin ≤40 g/L, vascular access and dialysis vintage (<2 years, 2–5 years and >5 years) will be performed for exploratory purposes. Assumptions and fits of the statistical models will be evaluated using standard approaches.…”
