Background: The objectives of the study were to explore a self-report measure for psychological well-being and to investigate the relationship between psychological well-being and psychological distress. Method: Telephone interviews of a representative sample of adults (N = 1933) collected information about sociodemographic variables, a standardised measure of psychological distress, and three brief existing scales to assess aspects of psychological well-being: Positive Relations with Others, Environmental Mastery, and Satisfaction with Life. The total of these three scales was also computed and explored as a measure of overall well-being. Results: Variables positively associated with psychological well-being were negatively associated with psychological distress and vice versa. For example low psychological well-being and high psychological distress were associated with being the only adult in the household, speaking a language other than English at home, being divorced or separated, having no educational qualifications beyond secondary school, being unable to work, having a low income, renting one's accommodation, and receiving a pension. Conclusions: The measure of well-being shows psychometric promise for community surveys. Psychological well-being is not exactly the opposite end of the continuum to psychological distress, but more debate is needed about whether and when, research participants need to be asked questions about both.

Aims: Court liaison services aim to reduce mental illness in prison through early treatment and/or diversion into care of defendants negotiating their court proceedings. However, liaison services may inadvertently contribute to gender inequalities in mental health in the prison system because women often do not access liaison services. This is attributed to services failing to recognize that women have different needs from men. To address this, it is essential that the needs of women in contact with the criminal justice system (CJS) are clearly articulated. However, there is a dearth of research that considers women’s needs at this stage of their journey through the CJS. This paper aims to identify these needs before women enter prison. It does so through an analysis of a pilot Women’s Support Service based at a magistrates’ court, a response to concerns that women were not accessing the local liaison service. Methods: Proformas were completed by a women’s specialist worker for 86 women defendants assessed over four months. Information was collected on characteristics including education, domestic violence, accommodation, physical and mental health. This specialist worker recorded the range of needs identified by defendants at assessment and the services to which women were referred. Results: Access to the Women’s Support Service is high, with only 11.3% of women refusing to use the service. Women attending have high levels of physical and mental health issues. Their mental health issues have not being addressed prior to accessing the service. Women often come from single households and environments high in domestic abuse. Women have multiple needs related to benefits, finance, housing, domestic abuse, education and career guidance. These are more frequent than those that explicitly link to mental health. The women’s worker providing the service referred women to 68 services from a wide variety of statutory and voluntary organizations. Conclusions: The Women’s Support Service is accessed by a higher number of women, many more than access the local liaison service. It is suggested that this is due to their multiple and gender-specific needs being adequately addressed by the former service and the organizations to which they are referred. Mental health needs may also be secondary to other more basic needs, which makes the generic service provided by the Women’s Support Service more appropriate than a liaison service that deals with mental health support alone.

El interés hacia la promoción y prevención en la salud mental ha aumentado en las últimas décadas acorde al incremento en la prevalencia e incidencia de las enfermedades mentales, pero paradójicamente no ha habido avances significativos en la promoción y prevención en salud mental, por lo que podemos decir que viene arrastrando un estado de deficiente implantación y de permanente crisis, sin que hayan existido claros esfuerzos dirigidos a desarrollar programas preventivos eficaces ni apoyos para potenciar la investigación en este área. Algunos han tratado de minimizar este estado de deficiencia de la prevención en salud mental desprendiendo una actitud de optimismo al alegar que existe suficiente evidencia para demostrar que los programas de prevención y promoción de salud mental funcionan y contribuyen a un incremento del bienestar mental y a una mejoría de la calidad de vida individual y comunitaria (1; 2), pero esto no es del todo cierto.Todo sistema sanitario tiene como misión fundamental ofrecer a sus ciudadanos el mayor nivel deseable de salud. Para conseguir este objetivo es necesario proponerse alcanzar al menos dos metas: el establecimiento de medidas preventivas adecuadas, individuales o comunitarias, y la aplicación de medidas terapéuticas. Pero el problema del sistema sanitario vigente es el desequilibrio hacia un mayor desarrollo del tratamiento de las enfermedades mentales en detrimento de una cobertura preventiva adecuada y eficaz para dichas enfermedades.El objetivo de este artículo es realizar una revisión de la literatura que nos ofrezca una visión global del estado actual de la promoción y prevención en salud mental. En primer lugar, vamos a analizar la evolución histórica en las últimas décadas y el contexto en el que está inmersa para poder así describir en qué punto se encuentra hoy en día. A partir de ahí, se evaluarán algunas de las cuestiones abiertas todavía hoy con respecto a la prevención en salud mental y los diversos problemas que podría conllevar puesto que no está exenta de riesgos. Y, por últi-mo, se realiza el planteamiento de diversas propuestas en un futuro próximo dirigidas a mejorar la prevención primaria y promoción de la salud mental. RESUMEN: Análisis y evolución histórica en las últimas décadas en prevención primaria y promoción de la salud. Propuestas para investigaciones futuras.
