The natural disease course of ankylosing spondylitis

Abstract: One hundred fifty war veterans with ankylosing spondylitis were entered into a prospective study in 1947. In 1957, 142 were traced, and they have been reviewed periodically. Eighty‐one of these patients were still alive in 1980. Information was obtained from 67 (83%) of the survivors and 51 were reexamined. “…This is reflected by its inclusion in the modified New York Criteria for the classification of ankylosing spondylitis (AS) and in the core set of domains for the evaluation of patients with SpA in clinical practice and trials, as defined by the Assessment of SpondyloArthritis international Society (ASAS) (1). A study on the natural course of AS conducted in the pre-biologic agent era described that in the long term more than half of all patients will develop moderate to severe impairment of spinal mobility (2). Spinal mobility is one of the central outcomes included in observational and treatment studies in axial SpA (3,4) and is a predictor of poor prognosis (3).…”

“…[5] It mostly affects men, especially in their sexually active ages. Some studies have suggested that progression in AS is most rapid in the first 10 years of the disease, [6,7] and this fact can greatly influence sexual activity. Otherwise, sexual problems created by both physical changes of the illness and its attendant distress not only affect these patients, but also their partners.…”

“…It has been linked to HLA B-27. Extra-arti-cular involvement is frequent and includes anterior uveitis, cardiac manifestations, pulmonary fibrosis, arachnoiditis, 9 cauda equina syndrome, and amyloidosis [10][11][12] . It also has been associated with ulcerative colitis, regional enteritis, psoriasis, multiple sclerosis, Reiter's disease, and Behçet's disease.…”
