The morbidly adherent placenta

Abstract: Women with an antenatal diagnosis of placenta accreta should be managed in a tertiary facility with multidisciplinary input. To determine optimum management strategies, it is imperative that larger studies are carried out in the future. It is essential that the continual monitoring and containment of rising caesarean section rates becomes a priority to prevent a further increase in the incidence of placenta accreta.

“…Attempt to remove the placenta leads to disruption of the infiltrated bed and is associated with increased maternal morbidity. It has been suggested that it may be sensible to complete the delivery of the infant and proceed to an intended hysterectomy with the placenta left in situ 13,29,38,41,42. A total rather than subtotal hysterectomy is advocated because of the risk of hemorrhage from lower-segment invasions.…”

“…During pregnancy, the vascular resistance in the uteroplacental circulation decreases, and; therefore, a persistently low PI after delivery would theoretically reflect the persistence of the placenta. 52,53 The PI on the side of the placental retention is also found to be lower than on the contralateral side. 53 The evidence regarding the efficacy of PI in the conservative management is perplexing.…”

“…The measurement of the PI in uterine arteries has also been proposed. During pregnancy, the vascular resistance in the uteroplacental circulation decreases, and; therefore, a persistently low PI after delivery would theoretically reflect the persistence of the placenta 52,53 . The PI on the side of the placental retention is also found to be lower than on the contralateral side 53 .…”

“…Recent studies have reported that pelvic artery catheterization and embolization are safe and effective to prevent HE in women with PAS [25,26]. A study by Munoz et al in 2023 demonstrates that UAE increases perioperative outcome and reduces maternal morbidity.…”
