The morbidity of laparoscopic radical cystectomy: analysis of postoperative complications in a multicenter cohort by the European Association of Urology (EAU)-Section of Uro-Technology

Abstract: In this multicenter, prospective, large database, LRC appears to be a safe but morbid procedure. Standardized complication reporting should be encouraged to evaluate objectively a surgical procedure and permit comparison across studies.

“…In the present study, however, overall and high‐grade complication rates were 53% and 17%, respectively, which were almost comparable with a recent ORC series . Consistent with the present results, recent randomized, controlled trials for RARC in addition to recent multicenter RARC and LRC studies have shown that the early complication rates were similar to that for ORC.…”

“…Several publications, including systemic reviews, have shown favorable postoperative complication rates for LRC or RARC, but most of the available studies are single high‐volume center experiences and therefore suffer from patient selection bias . Therefore, multi‐institutional databases are desirable for analyzing the outcome of minimally invasive radical cystectomy, but they are currently very limited . In addition, most studies of minimally invasive radical cystectomy were carried out in high‐volume academic Western institutions, and data from Japan and other Asian countries are scarce.…”

“…As a treatment for muscle invasive bladder cancer, radical cystectomy remains a highly morbid operation with complication rates of 40-60% and mortality rates as high as 9% in the first 90 days after surgery. [19,20] Albisinni et al [21] have analyzed postoperative complications after laparoscopic radical cystectomy (LRC) and evaluated its risk factors in a large prospective cohort built by the ESUT across European centers involved in minimally invasive urology in the last decade. A total of 548 patients were available for final analysis, of which 258(47%) experienced early complications during the first 90 days after LRC.…”

“…One of the most surprising results of this report was the high incidence of vaginal cuff dehiscence, which occurred in seven of 100 female patients. The incidence of this disastrous complication seems to be too high compared with previous reports . It is difficult to realize that this complication is not uncommon, and some critical questions as to whether this complication occurred in particular institutions, whether authors also experienced this complication when using open surgical procedure or what kind of surgical technique was used for these patients remain.…”

“…Ureteroileal anastomotic and urethral anastomotic strictures, and abdominal wall‐related complications were commonly observed. Another report of laparoscopic radical cystectomy showed late complications were observed in 12% of patients, and mainly involved ureteroileal anastomotic stenosis or incisional hernias . Hautmann et al .…”
