The article describes assessments and measurements performed in the Brazilian Longitudinal Study for Adult Health (ELSA-Brasil). Some assessments including anthropometric assessment, casual blood pressure measurement, and anklebrachial index have an established clinical application while others including pulse wave velocity, heart rate variability, and carotid intima-media thickness have no established application and do not have reference values for healthy Brazilian population but may be important predictors of cardiovascular outcomes. Blood pressure measurement following postural change maneuver was included in the ELSA-Brasil because it has not been much tested in epidemiological studies. Innovative approaches were developed for assessing the ankle-brachial index using an automatic device instead of the mercury column to measure blood pressure and for assessing the anterior-posterior diameter of the right lobe of the liver by ultrasound for quantitative assessment of nonalcoholic fatty liver disease. All ELSA-Brasil subjects were younger (35 years or more) than those included in other cohorts studying subclinical atherosclerosis. The inclusion of younger individuals and a variety of assessments make the ELSA-Brasil a relevant epidemiology study nationwide and worldwide.

Different effects of apolipoprotein A5 SNPs and haplotypes on triglyceride concentration in three ethnic origins

Several polymorphisms in the ApoA5 gene emerged as important candidate genes in triglyceride metabolism. The aim of this study was to determine the associations between ApoA5 polymorphisms, plasma triglyceride concentrations and the presence of cardiovascular disease (CVD) in three ethnic origins. Genotypes for 15 single nucleotide polymorphisms (SNPs) were determined in 659 older adults (mean age 71 ± 7 years) who immigrated to Israel or whose ancestors originated from East Europe (Ashkenazi), North Africa, Asia (Sephardic) or Yemen (Yemenite). The minor alleles of the four common SNPs (rs662799, rs651821, rs2072560 and rs2266788) are associated with an increase of 27-38% in triglyceride concentration among Ashkenazi and Yemenite Jews compared with the major alleles, but not among those of Sephardic origin. Conversely, among the Sephardic group, the presence of the minor allele in SNP rs3135506 compared with the major allele was associated with an increase of 34% in triglyceride concentration. The four SNPs were in significant linkage disequilibrium (D¢¼0.96-0.99), resulting in three haplotypes H1, H2 and H3, representing 98-99% of the population. Haplotype H2 was significantly associated with triglyceride concentration among Ashkenazi and Yemenite but not among Sephardic Jews. Conversely, haplotype H3 was associated with triglyceride concentration in Sephardic but not in Ashkenazi and Yemenite Jews. Ashkenazi carriers of H2 haplotype had a CVD odds ratio of 2.19 (95% CI: 1.05-4.58) compared with H1 (the most frequent), after adjustment for all other risk factors. These results suggest that different SNPs in ApoA5 polymorphisms may be associated with triglyceride concentration and CVD in each of these ethnic origins.

Prevalence of Preexisting Metabolic Syndrome as Defined by Japanese Original Criteria among Patients with Non-fatal Myocardial Infarction

Objective We aimed to clarify the prevalence of preexisting Metabolic Syndrome (MetS) defined by the Japanese original criteria among patients with non-fatal myocardial infarction (MI). Methods This is a retrospective cohort study using the computer database obtained by the preliminary health checkup from April 2003 to December 2008. We extracted the subjects with newly developed nonfatal MI from the study population. The newly non-fatal MI was diagnosed by the history of coronary heart disease (CHD) and new appearance of abnormal Q wave on electrocardiograms. MetS was diagnosed by using the Japanese original criteria. If waist circumference was not available, BMI was used alternatively. We evaluated the prevalence of preexisting MetS and other risk factors of CHD among the subjects. We compared the prevalence of preexisting risk factors between MetS group and non-MetS group. Results From a study population of 298,455 subjects, 446 subjects with a history of CHD were found. Among the 446, 92 subjects (85 men and 7 women) with abnormal Q wave on electrocardiogram were found. The prevalence of preexisting MetS with non-fatal MI was 19.6% (95% CI; 15.5-23.7%). The prevalence of other preexisting risk factors were 60.0% with smoking history, 55.6% with over-work, 53.3% with stressful life and 36.1% with impaired glucose tolerance. These prevalence rates were not significantly different between MetS group and non-MetS group. Only the prevalence (22.3%) of elevated LDL-cholesterol in the non-MetS group was significantly higher than in the MetS group (14.4%). Conclusion Preexisting MetS may be able to predict only 20% of future MI. To prevent future myocardial infarction, precaution guidance may be required for people with not only preexisting MetS but also other preexisting risk factors of CHD.
