Introduction: Breast cancer constitutes 10.9% of all new cases of tumours registered in the Polish National Cancer Registry. Post-mastectomy lymphoedema of the upper limb is the most common post-operative complication. The aim of the study was to evaluate the effectiveness of selected physiotherapeutic methods in the treatment of post-mastectomy lymphoedema. Material and methods: The study included 90 post-mastectomy patients in Stalowa Wola, Zamość and Tyczyn in the period from January 2014 to July 2016. The patients participated in two-week outpatient physiotherapy. In order to assess the effectiveness of lymphoedema treatment, circumferences of upper extremities were measured before and after physiotherapy. The results were then statistically analysed. Results: The majority of the subjects had undergone mastectomy within a period shorter than 5 years before. It was noted that the post-mastectomy upper limb had a significantly smaller circumference after physiotherapy than before physiotherapy (p<0.001). The study revealed no differences in the measurements of arm circumference with regard to the time since the mastectomy was performed. According to the patients, manual lymphatic drainage was the most effective type of physiotherapy treatment (p=0.04). Statistical analysis revealed significantly smaller arm circumferences after physiotherapy than before physiotherapy in patients treated 2-3 times (p=0.001) compared to patients who underwent either fewer or more treatments at the same time. Conclusions: 1. Two-week physiotherapy treatment proved to be the most effective in reducing lymphoedema in the arm area. 2. The period since the performance of the mastectomy had no impact on the effectiveness of lymphoedema treatment. 3. According to the post-mastectomy patients, manual lymphatic drainage was the most effective type of lymphoedema treatment. 4. Applying 2-3 physiotherapeutic procedures at the same time showed the best results.

Wpływ kompleksowej terapii udrażniającej na redukcję wtórnego obrzęku chłonnego u kobiet po leczeniu z powodu raka piersiThe effects of complete decongestive therapy on the extent of secondary lymphoedema in women after breast cancer treatment StreszczenieCel pracy: Celem pracy była ocena kompleksowej terapii udrażniającej (KTU) w redukcji wtórnego obrzęku chłonne-go u kobiet po leczeniu z powodu raka piersi. Materiał i metody:Badaniami objęto 20 kobiet po leczeniu z powodu raka piersi w średnim wieku 63,15 roku (± 8,15 roku), które zostały podzielone na dwie grupy: badaną (n = 10) i kontrolną (n = 10). Kryterium podziału stanowiło występowanie wtórnego obrzęku chłonnego. Kobiety z grupy badanej poddano 10 zabiegom KTU. U 70% badanych przeprowadzono radykalną mastektomię, a u 30% -zabieg oszczędzający z limfadenektomią pachową. Dodatkowo u 60% włączono radioterapię, u 70% -chemioterapię oraz u 80% -hormonoterapię. Średni czas po leczeniu wynosił 6,2 roku (± 3,5 roku). W grupie kontrolnej nie stosowano żadnych zabiegów przeciwobrzękowych. U 90% wykonano mastektomię, a u 10% -zabieg oszczędzający. Dodatkowo 40% kobiet przeszło leczenie uzupełniające w postaci radioterapii, 80% -w postaci chemioterapii oraz 50% -w postaci hormonoterapii. Średni czas po leczeniu wynosił 6,3 roku (± 4,4 roku). Podczas badań wykonano pomiar wielkości obrzęku chłonnego, wykorzystując taśmę centymetrową oraz program Limb Volumes Professional 5.0. Wyniki: W przypadku grupy poddanej zabiegom KTU uzyskano istotną redukcję w zakresie wielkości obrzęku chłon-nego (p = 0,005), której nie zaobserwowano w grupie kontrolnej (p = 0,33). Wnioski: Kompleksowa terapia udrażniająca stanowi skuteczną metodę postępowania przeciwobrzękowego u pacjentek z wtórnym obrzękiem chłonnym po leczeniu z powodu raka piersi. Introduction: Breast cancer is the most common malignant tumor in women in the Polish region. Surgery is a basic method of breast cancer treatment. Surgery often carries a lot of unwanted changes as follows: limitation of mobility in the shoulder joint on the operated side, secondary lymphoedema, post mastectomy pain syndrome (PMPS), reduction of muscle strength or disorders in body posture. Therefore, the implementation of physiotherapeutic activities that are designed to prevent and eliminate postoperative complications seems very important. The main aim of this work was to present physiotherapeutic management in women after mastectomy based on the analysis of available literature. The physiotherapeutic process can be divided into three periods: early hospital, early ambulatory and the late ambulatory period. In the first period, active slow exercises, self-support of the upper limb on the operated side and breathing exercises on the thoracic track are used to prevent circulatory disorders, pulmonary complications, and edema. The early ambulatory period includes corrective exercises, general improvement exercises, stretching and learning of automatic massage of the upper limb of the operated side. The last period should be enriched by recreational methods of physical activity such as swimming, cycling or Nordic walking to maintain physical fitness, proper mobility of the shoulder girdle and improve the patient's psychophysical state. It is very important the patient regular continues the rehabilitation program after curing of breast cancer as well. In the case of secondary lymphoedema of the upper limb, comprehensive rehabilitation physiotherapy is used, consisting of manual lymphatic drainage, healing exercises, compression therapy, and skin care. Conclusions: Physiotherapy in women after breast cancer surgery is a complex and long-term process. Physiotherapeutic methods are effective in treating complications after surgery of breast cancer surgery. It is necessary to constantly update the physio-therapy knowledge in women after breast cancer surgery.
