The measurement of haemoglobin A1c by isoelectric focussing in diabetic patients

Abstract: Summary. Incubation in vitro of human red cells in increasing glucose concentrations results in a rise in both haemoglobin A1~ levels and an intermediate band when measured by an isoelectric focussing method. There are strong correlations between blood glucose levels, levels of haemoglobin Alc and the intermediate band both in vitro and in blood samples taken from diabetic patients. As the intermediate band is also included in the measurement of haemoglobin A~c by the usual analytical methods, this may lead to… Show more

“…We believe that it is especially important to eliminate the labile component before HbA 1c estimation (15)(16)(17)(18)(19)(20), although this idea is not universally accepted (39-41). Some methods, e.g., affinity chromatography, isoelectric focusing, and thiobarbituric acid colorimetry, are not affected by labile HbA, c (11)(12)(13)(14). In the most commonly used method based on charge differences, elimination of labile HbA lc is achieved very simply with our method.…”

“…There are many methods of estimating HbA lc . Although some measure only stable HbA lc and some separate out labile HbA lc , most of these methods are impractical for routine use (11)(12)(13)(14). Cation-exchange chromatography is the most commonly used laboratory method.…”

“…The association of retinopathy and Albustixpositive proteinuria has long been recognized and it has been demonstrated that those Type 1 diabetic patients with an albumin excretion rate above 15 ILg/min are more likely to progress to proliferative retinopathy" as well as to persistent proteinuria. Our findings are consistent with this.…”
