“…1 This unprecedented therapeutic intervention was the scenario that allowed the description of acute respiratory distress syndrome (ARDS), the therapeutic benefit of the use of positive end-expiratory pressure (PEEP), alveolar recruitment and prone ventilation. “…1 Esta intervención terapéutica sin precedentes fue el escenario que permitió describir el síndrome de insuficiencia respiratoria aguda (SIRA), el beneficio terapéutico de la aplicación de la presión positiva al final de la espiración (PEEP), el reclutamiento alveolar y la ventilación en prono. [1][2][3][4][5][6][7] Debido a que la insuficiencia respiratoria es siempre la primera falla orgánica en el paciente con síndrome de falla orgánica múltiple, disponer de suficientes ventiladores es de importancia capital, en particular en momentos aciagos como los que estamos viviendo. Ante la posibilidad de que numerosos pacientes presenten insuficiencia respiratoria aguda en México por la pandemia de Covid-19 y la necesidad de aplicar un protocolo de ventilación mecánica masiva a "víctimas" debido a déficit de ventiladores, una alternativa viable puede ser la ventilación simultánea a varios pacientes con un solo ventilador, sobre todo a quienes compartan características fisiopatológicas similares.…”
