Spermatic cord leiomyosarcomas (LMSs) are rare tumors which may cause significant morbidity and mortality if inadequately diagnosed or treated. We report a case of a paratesticular LMS in a 60-year-old man who presented with a right scrotal mass. The patient was evaluated by scrotal ultrasound and computed tomography of the abdomen and pelvis (including scans of the scrotum), which revealed a large extratesticular mass. The lesion proved to be malignant and the patient underwent radical orchiectomy with high cord ligation. To improve the assignment of this lesion, we further analyze the imaging features of LMS and correlate them with pathologic findings.

Первичные паратестикулярные опухоли составляют около 7-10% всех внутримошоночных опухолей. У взрос-лых большинство этих поражений (до 75%) возникает из семенного канатика. Таким образом, первичные опухоли семенного канатика являются наиболее распространен-ными опухолями паратестикулярной области и представ-ляют большой клинический интерес [1,2]. Тем не менее их распознавание вызывает затруднения из-за относи-тельно редкой встречаемости.Наиболее полный описательный анализ опухолей се-менного канатика выполнен в США в рамках программы SEER (Surveillance, Epidemiology and End Results) -про-граммы Национального института рака США, крупней-шего реестра информации о распространении, течении и исходах злокачественных новообразований, который ох-ватывает около 28% населения. SEER собирает и публику-ет данные из 20 популяционных регистров рака и служит ценным источником для выполнения описательного ана-лиза редких злокачественных новообразований [3]. Дан-ная программа была использована для выявления всех случаев заболевания злокачественными опухолями се-менного канатика с 1973 по 2007 г. в США. За этот период в базе данных SEER зарегистрировано 362 случая опухо-лей семенного канатика.Общая частота этих новообразований составляет около 0,3 случая на 1 000 000 населения. Выявлены зна- Представлен обзор литературы о первичных паратестикулярных опухолях. Описаны классификация, краткая характери-стика липосарком и морфологическая картина опухоли с иммуногистохимическим профилем. Paratesticular leiomyosarcoma treated by enucleation

Spermatic cord leiomyosarcomas are rare tumors and standard treatment consists of radical orchiectomy and high cord ligation. We report a case of a paratesticular leiomyosarcoma successfully treated by enucleation. A 22-year-old man presented with a 6-months history of inguinal pain. Physical examination revealed a right paratesticular nodule about 0.5 cm in diameter. Inguinal exploration and nodule biopsy were performed. It was thought to be a benign epididymal nodule on a quick section and the tumor was enucleated and sent for paraffin section. Histology and immunohistochemistry were compatible with leiomyosarcoma. The patient was advised to undergo radical orchiectomy with high cord ligation. However, he refused surgery. An alternative approach with clinical, biochemical and radiological follow-up was adopted. The patient has been followed up for thirteen years and shows no evidence of disease.

Abstract: In this series, relapse was common after orchiectomy alone. Adjuvant radiation therapy may reduce the incidence of locoregional failure.
