The management of myocarditis

Abstract: Despite considerable advances in our understanding of myocarditis pathogenesis, the clinical management of myocarditis has changed relatively little in the last few years. This review aims to help bridge the widening gap between recent mechanistic insights, which are largely derived from animal models, and their potential impact on disease burden. We illustrate the pathogenetic mechanisms that are prime targets for novel therapeutic interventions. Pathway and pathogen-specific molecular diagnostic tests have e… Show more

“…At that time we did not have the possibility to carry out viral genome examinations, which probably explains the failure to attain a good long-term effect as it is likely that there were virus-positive cases among those we then treated. The importance of the virological diagnosis is highlighted by the fact that the subsequent therapy may depend on it [20][21][22]. A further reason why an effort should be made to reach an accurate aetiological diagnosis is the situation that the costs of the antiviral therapy applied in viral myocarditis are not negligible and, until the value of this therapy has been clearly proved by studies of large numbers of such patients, the lack of official financial support hampers its use in the everyday therapeutic practice.…”

“…О стрый миокардит является серьезным и по-тенциально фатальным заболеванием, однако заболеваемость и распространенность до настояще-го времени не уточнены [1]. Хотя до 40% больных с острым миокардитом выздоравливают спонтанно, у остальных наступает переход в хроническое тече-ние вследствие аномального иммунного ответа с со-хранением дилатации левого желудочка и сердечной недостаточности либо имеет место фатальный исход [2][3][4].…”
