Purpose Societal change and rise in demand for healthcare call for new health professional practices and task redistribution. Through negotiated order theory, this study explores how hospital rheumatologists (RT) and occupational therapists (OT) negotiate professional tasks in the clinical management of hand osteoarthritis. Methodology Fourteen qualitative interviews and 16 observations in clinical consultations were conducted in two hospitals specialized in rheumatology in Norway. Participants included eight OTs, six RTs, and patients in consultations. Data were analyzed using reflexive thematic analysis. Results Three themes were developed from codes: hierarchical ordering of hospital work impacts interprofessional negotiations; diagnostic organization of tasks preserves RT authority; and evidence-based recommendations in rheumatology enhance OT responsibilities. Overall, RTs and OTs enact tasks in succession where higher-ranking RTs establish a diagnosis and decide the subsequent in-hospital trajectory entrenched in a medical knowledge system. When medicine does not hold evidence-based treatment alternatives for patients, OTs respond by providing therapeutic interventions that are legitimized through international recommendations in rheumatology when they equip patients with tools to cope with chronic illness. Conclusion Negotiations over tasks do not take place from equal power positions when status and knowledge hierarchies frame professional practices. The enactment of tasks is concurrently highly influenced by the arena of the workplace, where the two professional groups both cross boundaries and work together in concert despite professional differences in order to meet patient interests and provide relevant healthcare.

How international are the editorial boards in the field of hand research? A cross-sectional study of leading subspecialty hand journals

Background Although low- and middle-income countries (LMIC) have great disease burden, but the lack of studies from LMIC have been shown in several fields. Multiple researchers from LMIC perceive editorial bias against their studies. Editorial board members (EBMs) from LMIC are under-represented across many medical journals. It is still unclear whether this phenomenon exists in the field of hand research. The purpose of this study was to investigate the composition of EBMs in leading subspecialty hand journals, and to reveal the international representation of EBMs in the field of hand research. Methods This cross-sectional study included seven leading subspecialty hand journals. The EBMs were obtained from the journals’ websites. The country affiliations of EBMs were categorized based on their locations and economy status. The composition of EBMs was investigated. Results There were 211 EBMs in the seven journals. A total of 185 EBMs (87.7%) were affiliated with high-income countries (HIC), 18 (8.5%) with upper middle-income countries, and 8 (3.8%) with lower middle-income countries. None EBMs were affiliated with low income countries. The EBMs were affiliated with 30 countries. The biggest number of EBMs were affiliated with the USA 74 (35.07%), followed by the United Kingdom (45, 21.33%), and France (13, 6.16%). Most of EBMs were based in Europe and Central Asia (86, 40.8%) and North America (81, 38.4%). Conclusions The EBMs of leading subspecialty hand journals are dominated by HIC with a very low representation of LMIC. There is a need to make the editorial boards more international in the field of hand research.

El osteoartriti (OA), birey ve toplum üzerinde önemli bir yük oluşturan dünya çapında sık görülen bir sağlık problemidir. Hastalık, el eklemlerini etkileyen lokal bir OA formudur. El OA prevalansı yaşla birlikte artmasına rağmen, orta yaşlı insanların çoğunda en az bir el eklemini etkileyen radyografik OA bulunmaktadır. Genel olarak hastaların % 3-16’sı semptomatik hale gelmekte ve hastalığın değişken bir klinik gidişatı bulunmaktadır. Ağrı, sertlik, fonksiyonel bozukluk sıklıkla bildirilen semptomlar olup sonuçta günlük yaşam aktivitelerinde kısıtlanma ve yaşam kalitesinde azalma oluşmaktadır. El OA tanısı klinik ve/veya radyolojik olarak konulmaktadır. Yüksek prevalansına rağmen, hastalığın yönetiminde etkili tedavi modaliteleri az sayıdadır. Son yıllarda el OA yönetimine daha fazla dikkat çekilmiştir. Avrupa Romatizma Karşıtı Birlik (European League Against Rheumatism-EULAR), Amerikan Romatoloji Koleji (The American College of Rheumatology-ACR)/Artrit Vakfı (Arthritis Foundation-AF) önerileri başta olmak üzere çok sayıda klinik rehber yayınlanmıştır. El OA farmakolojik ve non-farmakolojik tedavileri temel olarak; hasta eğitimi ve kendi kendine yönetimi, ortezler, egzersiz, fizik tedavi modaliteleri, topikal veya sistemik nonsteroidal antiinflamatuvar ilaçlar, oral kortikosteroidler ve farklı intra-artiküler tedavileri içermektedir. Literatürde yeni tedavi seçenekleri önerilmiş olmakla birlikte, bunlar hakkında yeterli kanıtlar bulunmamaktadır ve bu durum klinik uygulamada dikkatle değerlendirilmelidir. Bu makalede, konuyla ilgili sağlık profesyonellerine rehberlik etmesi amacıyla, el OA yönetiminde yer alan farmakolojik ve non-farmakolojik tedavi yaklaşımları ile birlikte yeni gelişmeler gözden geçirilmiştir.
