Over more than 60 years, monoamine oxidase (MAO) inhibitors are available for therapy of central nervous diseases. Although they have shown to be efficacious specifically in the treatment of major depressive disorders and treatment-resistant depression, they became less a therapeutic choice for the physicians mostly due to severe side effects, such as liver failure and hypertensive crisis associated specifically with the first generation of inhibitors. Nevertheless, this class of drugs is still being used for treatment specifically as more selective and reversible inhibitors became available and will provide clinicians with additional treatment options. The current review revisits monoamine oxidase inhibitors and their potential in the treatment of human diseases, such as anxiety, depression, mood and personality disorders, and pain and introduces current ideas and developments.

-Social anxiety disorder (SAD) is a marked and persistent fear of doing almost everything in front of people due to concerns about being judge by others. An up-to-date review is needed in order to reach a practical judgement of all psychopharmacological data. Case reports, open and double-blind trials with SAD were described and commented upon from a clinical point of view. The MEDLINE system was searched from 1975 to 2001. The references from the selected papers were also used as a source. MAOIs (fenelzine, tranylcypromine), reversible monoamino oxidase-A inhibitors (moclobemide, brofaromine), SSRIs (paroxetine, sertraline, fluoxetine, fluvoxamine) and some other antidepressants (venlafaxine, nefazodone) have proven effective in several studies with various methodologies. The MAOIs have more serious adverse effects and the SSRIs have the best tolerance. SSRIs are efficacious and the first choice of treatment.KEY WORDS: social anxiety disorder, social phobia, pharmacologic treatment. Muitos estudos clínicos têm sido conduzidos com o objetivo de se obter um tratamento eficaz. É necessária uma revisão atualizada para alcançar um julgamento clínico de todos os dados com antidepressivos. O sistema MEDLINE foi pesquisado no período de 1975 a 2001. As referências dos artigos consultados também foram utilizadas com fonte. Antidepressivos inibidores da monoamino oxidase (IMAO) (fenelzine, tanilcipromina), inibidores reversíveis da monoamino oxidase tipo A (RIMA) (moclobemida, brofaromina), antidepressivos inibidores seletivos de serotonina (ISRS) (paroxetina, sertralina, fluoxetina, fluvoxamina) e alguns outros (venlafaxina, nefazodone) têm demonstrado eficácia em inúmeros estudos com diferentes metodologias. Os ISRS são o grupo mais estudado com metodologia duplo-cega, com melhores resultados e com boa tolerância, sendo a primeira escolha no tratamento. PALAVRAS-CHAVE: fobia social, transtorno de ansiedade social, tratamento farmacológico.

Combined Therapy in Anxiety Disorders: Current Data and Recommendations for the Clinical Practice Method:Over the past couple of years there has been a definite increase in studies on the effects of combined behavioral and pharmacological treatments of anxiety disorders. But their results have been partially contradictory and the interpretation of outcomes has sometimes been controversial. This article reviews the results of controlled studies for the different subtypes of anxiety disorders. Results:Although a few studies with patients with panic disorder and agoraphobia show an earlier response to combined treatment compared to behavior therapy alone, altogether the results are inconsistent. The combination of an initial pharmacotherapy alone with additional behavior therapy before drug discontinuation is helpful to reduce the otherwise high relapse-rates. Sometimes, the subsequent treatment of non-responders in behavior therapy or psychopharmacotherapy with the alternative method is helpful. ConclusionsNo convincing evidence was found that the parallel combination is generally more effective in any of the anxiety disorders than the single treatment. In the individual case, the decision for or against a combined treatment is dependent on several factors which are only partly investigated in controlled clinical trials. Especially the severity of the anxiety symptoms, comorbid symptomatology and the number of treatment refusers and dropouts have to be mentioned in this context. These factors, the results of naturalistic studies and ‘clinical experience’ have influenced our conclusions for clinical practice from the controlled studies.
