The Long-Term Results of Mitral Valve Reconstruction for the "Floppy" Valve

Abstract: The myxomatous degenerated, prolapsed, or floppy mitral valve is the most common etiology of mitral regurgitation in North American populations. We performed mitral valve reconstruction for this diagnosis in 252 patients from 1984 to 1993. There were 165 males and 87 females ranging in age from 23 to 84 years (mean 64 years); 93 (37%) were > or = 70 years. One hundred eighty-six were New York Heart Association Functional Class III or IV and 29% (72) underwent concomitant coronary bypass operation. Operations i… Show more

“…Failure to add an annuloplasty to a MV reconstructive procedure is associated with an increased risk of late reoperation. 18,19) The annuloplasty ring size for patients who underwent chordal reconstruction was significantly larger compared to those who underwent leaflet resection. Postoperative transmitral gradients were lower in the chordal reconstruction group.…”

“…However, it has also been reported to be associated with many conditions including connective tissue disorders. [1][2][3] MVP is three-times more prevalent in patients with BJHMS than other patients and may be present in up to one third of all individuals with BJHMS. [7][8][9]29) However, there are a few reports with conflicting results on the incidence of BJHMS in MVP 15,16) and the relation between the echocardiographic features of mitral valves and elastic properties of the aortic wall and BHS in patients with both MVP and BJHMS.…”

“…1) However, MVP occurs quite commonly in heritable disorders of connective tissue that increase the size of the mitral leaflets and apparatus, including Marfan syndrome, 2) Ehlers-Danlos syndrome, 3) and pseudoxantoma elasticum. 4) Increased joint laxity in patients with MVP was first reported in 1976 5) and denied in 1977.…”

“…Its prevalence depends on the severity of heart failure, and rises approximately from 5% in the NYHA I functional class up to 50% in NYHA IV class 7 . Up to 50% of patients undergoing mitral valve surgery are diagnosed with AF prior to the surgery [8][9][10][11] .…”
