The levels of the leptin and adiponectin according to body mass index and their relationship with oxidative parameters

Abstract

ÖZETAmaç: Obezite ateroskleroz için önemli bir risk faktörüdür ve sıklığı hızla artmaktadır. Son çalışmalar obezitede ve obezite ile ilişkili komplikasyonlarda leptin ve adiponektinin önemli rol oynadığını düşündürtmektedir. Bununla birlikte etki mekanizmaları tam olarak ortaya konulamamıştır. Bu çalışmada leptin ve adiponektin düzeylerinin vücut kütle indeksi (VKİ) ve oksidatif parametreler ile olan ilişkisi araştırıldı. Yöntemler: Enine-kesitli gözlemsel çalışmaya sağlıklı bireyler arasında VKİ ölçütlerine göre VKİ >35 kg/m 2 olan (n=29, Grup 1), VKİ = 25-30 kg/m 2 arası (n=29, Grup 2) ve VKİ <25 kg/m 2 (n=29, Grup 3) olmak üzere toplam 87 olgu çalışmaya alındı. Leptin ve adiponektin düzeyleri, total antioksidan kapasite (TAK) ve total oksidan seviye (TOS) düzeylerine bakıldı. TAK ve TOS değerleri kullanılarak oksidatif stres indeksi (OSİ) hesaplandı. İstatistiksel analiz Ki-kare, Mann-Whitney U, tek yönlü ANOVA, Kruskal-Wallis, Pearson korelasyon ve çoklu regresyon analiz testleri ile yapıldı. Bulgular: Yaş ve cinsiyet dağılımı gruplar arasında benzerdi. TAK düzeyi grup 1'de en küçük iken grup 3'de en yüksekti. TOS ise grup 1'de grup 2 ve 3'e göre daha yüksek (p <0.05), OSİ grup 2 ve 3'de benzer (p=0.059), grup 1'de diğerlerine göre istatistiksel açıdan anlamlı derecede farklıydı (p<0.05). Leptin düzeylerinde grup 1'den grup 3'e doğru anlamlı azalma, adiponektinde ise artma izlenmekteydi. Leptin düzeyleri TOS (β=-1.123, %95 güven aralığı (GA)=-12.734-0.255, p=0.040), OSI (β=1.689, %95 GA=1.105-12.481, p=0.018) ve bel çevresi (β=-0.592, %95 GA= -0.630-0.134, p=0.003) ile diğer parametrelerden bağımsız olarak ilişki gösterirken, adiponektin düzeyleri herhangi bir parametre ile anlamlı ilişki göstermemekteydi. Sonuç: Sağlıklı kişilerde VKİ artış ile birlikte leptin artmakta, adiponektin ise azalmaktadır. Meydana gelen bu değişikliklerden TOS ve OSI'nin sorumlu olabileceğini düşündürmektedir.
