We examined the patterns of osteonal remodeling associated with the implantation of a screw in the femoral diaphysis of four mongrel dogs. The dogs were killed at intervals ranging from 11 to 93 days after implantation, and resorption spaces and tetracycline-labeled osteons were counted as functions of postimplant time and distance from the screw. The contralateral femur was used as a control. We found that implantation of the screw initiated a sequence of remodeling activity at the screw site; numerous resorption spaces were observed after 3 weeks, followed by numerous refilling osteons at 7 weeks. When sequential sections along the shaft adjacent to the screw were examined, it appeared that the increased resorption and refilling activity subsequently migrated away from the screw site, both proximally and distally. Also, both the implanted and control femurs exhibited occasional zones along the shaft within which the numbers of labeled osteons were sharply reduced. These "extinction zones" were found to be associated with local maxima in resorption space density. We further noted that the ratio of singly to doubly labeled osteons in the experimental control femurs was high as compared with that in humans, but similar to that previously observed in the canine iliac crest.

Patients with Type 1 Diabetes Mellitus (DM) have markedly increased risk of fracture, but little is known about abnormalities in bone micro-architecture or remodeling properties that might give insight into the pathogenesis of skeletal fragility in these patients. We report here a case-control study comparing bone histomorphometric and micro-CT results from iliac biopsies in 18 otherwise healthy subjects with Type 1 Diabetes Mellitus with those from healthy age- and sex- matched non-diabetic control subjects. Five of the diabetics had histories of low-trauma fracture. Transilial bone biopsies were obtained after tetracycline labeling. The biopsy specimens were fixed, embedded, and scanned using a desktop μCT at 16 micron resolution. They were then sectioned and quantitative histomorphometry was performed as previously described by Recker et al. 1988.[1] Two sections, >250 μm apart, were read from the central part of each biopsy. Overall there were no significant differences between diabetics and controls in histomorphometric or micro-CT measurements. However, fracturing diabetics had structural and dynamic trends different from nonfracturing diabetics by both methods of analysis. In conclusion, Type 1 Diabetes Mellitus does not result in abnormalities in bone histomorphometric or micro-CT variables in the absence of manifest complications from the diabetes. However, diabetics suffering fractures may have defects in their skeletal microarchitecture that may underlie the presence of excess skeletal fragility.

The use of fluorochromes in bone research is a widely accepted technique that dates back to the 1950s. Several pioneers, such as Harold Frost, have thoroughly investigated the potential of fluorochrome use for the study on bone formation and bone remodeling dynamics. Since the development of bone tissue engineering, a renewed interest in the benefits of fluorochrome use was perceived. Fluorochrome use in animal models makes it possible to determine the onset time and location of osteogenesis, which are the fundamental parameters in bone tissue engineering studies. There is, however, a lack of standardized procedures for using this technique. In addition, many types of fluorochromes exist and one could be confused upon selecting the appropriate type, the appropriate concentration, the route of administration, and methods of visualization. All these variables can potentially affect the outcome during fluorescence microscopy. This work aims at providing the bone tissue engineering researcher with an overview of the history, working mechanism, and the potential pitfalls in the use of fluorochromes in animal studies. Experiments using some of the more frequently used fluorochromes are explained and illustrated.
