The Kinetics of 5-Fluorocytosine Elimination in Man

Abstract: Summary: 5‐Fluorocytosine (5‐FC) represents an important advance in the chemotherapy of cryptococcosis and some serious fungal diseases, especially systemic moniliasis. The drug is excreted mainly by the kidney and has a wide margin of safety, but potentially toxic high plasma levels follow normal dosage regimes in patients with renal insufficiency. Using 5‐fluorocytosine‐2‐14C, the plasma and urinary clearances of 5‐FC have been measured together with simultaneous inulin clearances. In eight healthy subjects… Show more

“…In addition, amphotericin B itself results in impairment of renal function with great regularity (6). 5FC is excreted almost exclusively by the renal route (2,20); elevated serum levels occur with renal failure (2,3,16). Although bone marrow suppression occurs infrequently with 5FC therapy, its occurrence may be related to the presence of inordinately high serum levels of 5FC (2,5,14).…”

“…Renal function assumes a very important role in determining serum levels and subsequent toxicity of 5-FC since this drug is excreted almost wholly by the kidneys (16). In the face of rapidly progressive renal failure, as in patient 1, very high serum levels of 5-FC can occur and persist for several days even though the drug has been discontinued.…”

“…To avoid toxicity during combined therapy, 5-FC serum concentration and serum creatinine should be monitored closely and the dosage of 5-FC adjusted accordingly. Several methods of adjusting the dosage of 5-FC on the basis of creatinine clearance have been proposed (12,16). Use of such formulas combined with determination of 5-FC serum concentrations should obviate the problem of 5-FC-related marrow toxicity.…”

“…Because of its nephrotoxic effects, administration of AmB frequently leads to a diminished creatinine clearance (10). 5-FC is excreted almost entirely by the kidneys (12). In the face of decreasing renal function, excessively high levels of 5-FC may accumulate.…”
