Background: Italy has been one of the countries most affected by the SARS-CoV-2 pandemic and the regional healthcare system has had to quickly adapt its organization to meet the needs of infected patients. This has led to a drastic change in the routine management of non-communicable diseases with a potential long-term impact on patient health care. We investigated the management of non-COVID-19 patients across all medical specialties during the pandemic in Italy. Methods: A PRISMA guideline-based systematic review of the available literature was performed using PubMed, Embase, and Scopus, restricting the search to the main outbreak period in Italy (from 20 February to 22 June, 2020). We selected articles in English or Italian that detailed changes in the Italian hospital care for non-COVID-19 patients due to the pandemic. Our keywords included all medical specialties in combination with our geographical focus (Italy) and COVID-19. Findings: Of the 4643 potentially eligible studies identified by the search, 247 studies were included in the systematic review. A decrease in the management of emergencies in non-COVID patients was found together with an increase in mortality. Similarly, non-deferrable conditions met a tendency toward decreased diagnosis. All specialties have been affected by the reorganization of healthcare provision in the hub-and-spoke system and have benefited from telemedicine during the pandemic. Interpretation: Our work highlights the changes taking place in the Italian public healthcare system in order to tackle the developing health crisis due to the COVID-19 pandemic. The findings of our review may be useful to analyze future directions for the healthcare system in the case of new pandemic scenarios.

In general, during the COVID-19 pandemic there has been a growth in the use of digital technological solutions in many sectors, from that of consumption, to Digital Health and in particular to mobile health (mHealth) where an important role has been played by mobile technology (mTech). However, this has not always happened in a uniform way. In fact, in many cases, citizens found themselves unable to take advantage of these opportunities due to the phenomenon of the Digital Divide (DD). It depends on multifaceted aspects ranging from the lack of access to instrumental and network resources, to cultural and social barriers and also to possible forms of communication disability. In the study we set ourselves the articulated goal of developing a probing methodology that addresses the problems connected to DD in a broad sense, capable of minimizing the bias of a purely electronic submission and evaluating its effectiveness and outcome. At the moment, we have submitted the survey both electronically (with an embedded solution to spread it inside the families/acquaintances) and using the wire phone. The results highlighted three polarities (a) the coherence of the two methods; (b) the outcome of the entire submission in relation to key issues (e.g., familiarity on contact tracing Apps, medical Apps, social Apps, messaging Apps, Digital-health, non-medical Apps); (c) a Digital Divide strongly dependent on age and in particular for the elderly is mainly evident in the use of mTech in general and in particular in mHealth applications. Future developments of the study foresee, after adequate data-mining, an in-depth study of all the aspects proposed in the survey, from those relating to access to resources, training, disability and other cultural factors.

Spain was one of the epicenters of the first wave of the COVID-19 pandemic. We describe in this article the design and results of a new telephone-and-telematic multiplatform model of systematic prenatal and postpartum follow-up for COVID-19-affected women implemented in a tertiary reference hospital in Madrid. We included patients with RT-PCR-confirmed COVID-19 during pregnancy or delivery from 10 March 2020 to 15 December 2020. We had a total of 211 obstetric patients: 148 (70.1%) were tested at the onset of suspicious clinical manifestations and 62 (29.4%) were tested in the context of routine screening. Of all the patients, 60 women (28.4%) were asymptomatic and 97 (46%) presented mild symptoms. Fifty-one women (24.2%) were admitted to our hospital for specific treatment because of moderate or severe symptoms. We had no missed cases and a good adherence. The mean number of calls per patient was 2.3. We performed 55 in-person visits. We analyzed the complexity of our program over time, showing a two-wave-like pattern. One patient was identified as needing hospitalization and we did not record major morbidity. Telemedicine programs are a strong and reproducible tool to reach to pregnant population affected by COVID-19, to assess its symptoms and severity, and to record for pregnancy-related symptoms both in an outpatient regime and after discharge from hospital.
