The intraoperative pathological findings in cases of chronic suppurative otitis media with central perforation of tympanic membrane at a tertiary care centre in Eastern Nepal

Abstract: Background: Chronic suppurative otitis media (CSOM) is the chronic inflammation of the middle ear cleft. The atticoantral type is called unsafe type because of the associated complications due to granulations and cholesteatoma which may be life-threatening. CSOM with central perforation (tubotympanic type) is usually not associated with major complications such as cholesteatoma formation.Aims and Objectives: To find out the intraoperative pathological findings in cases of CSOM with central perforation (tubotym… Show more

“…Thakur, et al, obtained similar results (43.6%). 10 The posterosuperior quadrant has more vascularization than other areas so if an inflammatory process occurs, it is susceptible to cell exposure. Repeated inflammation and negative intratympanic pressure effects tympanic membrane retraction until membrane rupture occurs, especially in areas with weaker structures.…”

“…The same results were obtained in the study by Thakur, et al, which stated that Granulation is formed due to an inflammatory process in the middle ear and causes bone erosion in both the ocular chain and the tympanic cavity wall. 10 The tympanoplasty technique used in this study was based on the Wullstein classification. Most types of tympanoplasty were type 1 (78.13%).…”

“…5 Though ossicular chain involvement is less common in COM mucosal, associated hearing loss may be due to perforation in the tympanic membrane or ossicular chain erosion/fixation (loss of ossicular coupling). 6,7 Simple closure of tympanic membrane in the presence of ossicular discontinuity or fixity will further decrease the quality of life due to long term acoustic failure in around 50 to 60% patients. 8 Pure Tone Audiometry (PTA), a routine preoperative procedure performed prior to myringoplasty, can help predict the severity of hearing loss and reflect the integrity of ossicular chain status.…”
