The Intracranial Vertebral Artery: A Neglected Species

Abstract: Among posterior circulation arteries, the intracranial vertebral artery (ICVA) has been given the least attention, especially concerning treatment of occlusive lesions. Early clinicopathological studies showed that the ICVA was often occluded in patients with lateral medullary and posterior inferior cerebellar infarcts. Severe stenosis or occlusion of the ICVA was the most common arterial lesion among the 408 patients in the New England Medical Center Posterior Circulation Registry (NEMC-PCR). In this registry… Show more

“…This is supported by findings from the New England Medical Center Posterior Circulation Registry, where almost half of the patients with a posterior circulation stroke had episodes of dizziness (with or without vertigo), and 5% of the patients had loss of consciousness [9]. More than half of the patients with an accompanying unilateral VA stenosis had complaints of lightheadedness [10]. Comparable frequencies were found in our study, where 71% of the patients with a posterior circulation event had nonrotatory dizziness or vertigo and 7% had loss of consciousness.…”

“…Severe intracranial vertebral artery stenosis (IVAS) or occlusion is the most common arterial lesion causing posterior circulation ischemia (Caplan 2012). The rate of recurrent stroke in patients with symptomatic intra-cranial atherosclerotic stenosis has been higher than 10% per year, despite aggressive medical treatment (Chimowitz et al 2005;Derdeyn et al 2014).…”

“…Intracranial vertebral artery (ICVA) is a common site of atherosclerosis, often involved bilaterally [1, 2]. Patients with bilateral ICVA occlusion usually present with recurrent attacks of dizziness, visual disturbance, and ataxia, and some may be disabled by infarction of regions supplied by the posterior circulation [2].…”
