This is a cross-sectional study of unmet needs of persons enrolled in Australia's Partners in Recovery (PIR) initiative. It aimed to explore the unmet needs reported by persons with a severe and persistent mental illness (SPMI) and to examine the associations between unmet accommodation needs and other unmet needs. The study was undertaken in the Gippsland region of Victoria from February to May 2015. Data were collected from the administrative database for the PIR initiative in Gippsland, which was held by the Gippsland Primary Health Network. Data on unmet needs, as measured by the Camberwell Assessment of Needs Short Appraisal Schedule, were analysed using proportions and logistic regression. Psychological distress, daytime activities, company/someone to spend time with and employment and volunteeringwere the most commonly reported unmet needs. Participants with unmet accommodation needs were less likely to receive information on their condition or access other services. They also had unmet needs relating to food, money, transport, childcare, looking after home, physical health, psychological distress and self-care.Supported accommodation may not be enough for persons with SPMI who have poor functioning skills and are incapable of looking after themselves. Services such as Housing First that have shown promising results need to be part of a comprehensive strategy to care for persons with severe and enduring mental illness.

Triangulation study of needs assessment of people with severe mental illness in “follow‐up” day hospital settings

Aim The aim of research was to assess the needs of patients with severe mental illness (SMI) attending follow‐up day hospital (DH) programmes from the patients', relatives' and experts' perspective. Design This triangulation research relies on three sources of information and two techniques of data collection. Methods Qualitative research was conducted comprising semi‐structured interviews (n = 15) and focus groups (n = 4) in three sequential rounds, triangulating patients', relatives' and experts' views. A content analysis was carried out. Results Study reveals a diverse spectrum of needs of SMI patients in follow‐up DH programmes. The analysis yielded six themes: optimal daily functioning, work and educational activities, social network inclusion, continuous treatment, support and guidance, long‐term care. DH programmes should assess the needs of patients with SMI and be adapted according to the findings. In addition to clinical treatment, requirements for continuous treatment, psychological and social needs, therapeutic relationship in less‐restrictive settings should be considered.

The important influence of the social determinants of health on the physical health status of individuals and communities is well established. Factors such as employment, education, and housing all make decisive contributions to the physical health a person enjoys. What is not as readily recognized is that the social determinants also have a very strong impact on a person's mental health. The ability of a person to live a contented and satisfying life has a lot to do with the conditions and circumstances in which they live, work, play, and age. Crucially, these social determinants are likely to affect people differentially with greater or lesser impacts in rural, remote, and very remote (RRvR) locations. Of particular importance in developing a nuanced understanding of the contributions the social determinants make to a person's mental state is appreciating the fundamental significance of control. Also crucial is a recognition of the inadequacy of linear cause-effect models and the necessity to incorporate bidirectional, mutually influencing models for a more sophisticated understanding of how people function within the environments they inhabit. After describing the way in which the social determinants might be considered in RRvR areas, the chapter concludes with an illustration of the way in which a case conceptualization might occur if the social determinants are thoroughly and genuinely incorporated.
