An ovarian fibroma is a rare entity and a diagnostic dilemma due to its solid nature and ultrasound findings being similar to a uterine fibroid. An ovarian fibroma, being parasitic, is extremely rare. We report the case of a 35-year-old, multiparous woman who presented with a 'wandering' abdominal mass of six months duration. Clinical examination revealed a 16-week size, solid, firm, well-defined mass in the right lumbar and iliac regions, separate from the uterus on bimanual examination, suggestive of a pedunculated subserous fibroid uterus. Ultrasound abdomen with color Doppler evaluation was suggestive of the nonuterine origin of the tumor, probably a gastrointestinal stromal tumor. In view of the diagnostic dilemma, MRI was done, which showed that the lesion had features characteristic of ovarian fibroma, however, bilateral ovaries were normally visualized. An exploratory laparotomy was performed. There was a solid parasitic tumor adherent to the bladder peritoneum and attached to the right ovary by a thin band, which was excised. Histopathology confirmed ovarian fibroma. To conclude, a parasitic ovarian fibroma is rare and a careful clinical approach with imaging and surgery helps in solving this diagnostic dilemma.

Ovarian masses, ranging from benign cysts to malignant tumors, present complex diagnostic challenges in women's healthcare. Early detection of ovarian masses is paramount for improving patient outcomes, as delayed diagnoses often lead to advanced-stage disease with limited treatment options. This comprehensive review explores screening methods' current state, limitations, and emerging technologies to facilitate earlier detection. The limitations of existing screening methods, such as low sensitivity and specificity, underscore the need for improved early detection strategies. Imaging-based techniques, including transvaginal ultrasound, magnetic resonance imaging, and computed tomography, are vital in evaluating ovarian masses. However, the emergence of artificial intelligence (AI) and machine learning (ML) applications enhances the accuracy of image interpretation. Blood-based biomarkers, such as CA-125, have been the focus of research for ovarian mass detection. While CA-125 remains widely used, its limitations have prompted investigations into alternative serum biomarkers, including HE4 and miRNA, along with liquid biopsy and circulating tumor DNA. Ultrasound-based scoring systems, such as the risk of malignancy index (RMI), Ovarian-Adnexal Reporting and Data System (O-RADS), and guidelines from the International Ovarian Tumor Analysis (IOTA) group, provide structured approaches for classifying ovarian masses. These systems aid healthcare providers in clinical decision-making. Emerging technologies, such as liquid biopsy, AI, and proteomic/metabolomic approaches, offer promising avenues for enhancing early detection and risk assessment. Liquid biopsy provides noninvasive, real-time monitoring of ovarian masses, while AI and ML applications improve the accuracy of image interpretation. Proteomic and metabolomic studies reveal novel biomarkers and molecular insights. High-risk populations, often associated with genetic mutations such as BRCA1 and BRCA2, require specialized screening strategies. Current guidelines recommend screening modalities, risk-reduction strategies, and shared decision-making. Ongoing research focuses on refining risk assessment and personalized screening for high-risk individuals. This review underscores the importance of early detection in managing ovarian masses, emphasizing the need for improved screening methods, tailored approaches for high-risk populations, and ongoing research to further enhance diagnostic accuracy and patient outcomes.

Background: The purpose of this study was to evaluate the ability of the International Ovarian Tumor Analysis-Assessment of Different NEoplasias in the adneXa (IOTA-ADNEX) model to distinguish between benign, borderline, and malignant epithelial ovarian tumors(BeEOTs, BEOTs, and MEOTs, respectively). Methods: The study included 813 patients with BeEOTs, BEOTs, and MEOTs who underwent ultrasound examinations and pelvic operations. Comparisons were made between the clinical information and ultrasonographic features of the three patient groups. Three clinical variables and six ultrasound variables were used to estimate malignancy risk. The sensitivity, specificity, positive predictive value, negative predictive value, and AUC (the area under the receiver operating characteristics [ROC] curve) of the ADNEX model were calculated. Results: Of the 813 patients, 257 (31.6%) had BeEOTs, 114 (14.0%) had BEOTs, and 442 (54.4%) had MEOTs. The most common type, serous and mucinous epithelial tumors, accounted for 81.3% of the total cases. In the MEOTs group, serous, mucinous, endometrioid, and clear cell tumors accounted for 85.3%, 2.5%, 4.5%, and 7.7% of the total cases, respectively. For a cut-off value of 10% to identify the overall risk for ovarian cancer (OC), the sensitivity and specificity were 99.1% and 73.2%, respectively. According to the ROC curves, the AUC was 0.987 (95% CI: 0.981–0.993) for BeEOTs compared with MEOTs, 0.820 (95% CI: 0.768–0.872) for BeEOTs compared with BEOTs, 0.912 (95% CI: 0.876–0.948) for BeEOTs compared with stage I OC, and 0.995 (95% CI: 0.992–0.998) for BeEOTs compared with stages II–IV OC. The AUC was 0.614 (95% CI: 0.519–0.709) for BEOTs compared with stage I OC, 0.903 (95% CI: 0.869–0.937) for BEOTs compared with stages II–IV OC, and 0.851 (95% CI: 0.800–0.902) for stage I OC compared with stages II–IV OC. Conclusions: The IOTA-ADNEX model demonstrated good diagnostic performance for the three categories of EOTs and can be helpful for clinical treatment management.
