Abstract. The objective of this research was to assess the effects of type 2 diabetes mellitus (T2DM) and diabetic nephropathy (DN) on the pharmacokinetics of human IgG (hIgG), an antibody isotype, in Zucker diabetic fatty (ZDF) rats. Furthermore, the specific role of T2DM in the altered disposition of hIgG was evaluated by treating diabetic rats with pioglitazone, while the role of chronic kidney disease (CKD) was assessed using 5/6 nephrectomized Sprague Dawley rats. ZDF male (lean non-diabetic control and obese diabetic) and pioglitazone-treated ZDF rats were studied at ages 12-13 weeks (only DM was present), and at ages 29-30 weeks (progression to DN). All animals were dosed with 1 mg/kg of hIgG intravenously (IV) or subcutaneously (SC). ZDF rats had significantly higher blood glucose concentrations and urinary albumin excretion compared to control rats. Significant increases in total clearance (2.5-fold) and renal clearance (100-fold) of hIgG were observed; however the major increase in total clearance was due to increased non-renal clearance. Greater changes in urinary albumin excretion and total and renal clearances of IgG (3.5-fold and 300-fold, respectively) were observed with progression to DN. SC bioavailability of hIgG in all animal groups was similar (>84%). With pioglitazone-treatment, diabetic animals remained euglycemic and treatment was able to reverse the clearance changes, although incompletely. In the CKD group, no difference in hIgG clearance was observed when compared with controls. In conclusion, the increased clearance of hIgG in ZDF diabetic animals, reversal by pioglitazone treatment and lack of effect of CKD, demonstrate the influence of T2DM on hIgG pharmacokinetics.

StreszczenieCukrzyca zaliczana jest do chorób cywilizacyjnych, ponieważ choruje na nią ok. 6,4% populacji w wieku 20-79 lat (w 95% przypadków jest to typ 2). W Polsce cukrzycę typu 2 stwierdzono u ponad 1,5 mln pacjentów, z czego 60% stanowią kobiety. Cukrzyca typu 2 długo nie daje żadnych objawów, często pozostaje nierozpoznana i nieleczona, prowadząc do nieodwracalnych w skutkach uszkodzeń narządów. Często jej pierwszym symptomem jest skłonność do ropnych zakażeń skórnych lub grzybicy. Według danych zespołu ekspertów Polskiego Towarzystwa Ginekologicznego (PTG), grzybice powierzchowne należą do najczęstszych zakażeń u kobiet. W terapii zapalenia grzybiczego sromu i pochwy (vulvovaginal candidosis -VVC) stosowane są leki z grupy azoli oraz antybiotyki polienowe. W leczeniu ostrych postaci VVC jak również w zapobieganiu nawrotom infekcji zalecane są pochodne triazolowe (flukonazol, itrakonazol). Wyniki ostatnio opublikowanych badań przemawiają za tym, że przedłużona terapia flukonazolem jest skuteczniejsza w leczeniu nawracających drożdżakowych zapaleń sromu i pochwy niż dotychczas stosowane "krótkie" schematy leczenia.Kolejnym istotnym powodem zgłaszania się kobiet chorych na cukrzycę do ginekologa jest problem doboru metod antykoncepcji. Jest on szczególnie istotny w tej grupie kobiet z uwagi na większe ryzyko zagrożenia dla kobiety i jej dziecka, kiedy ciąża jest nieplanowana. Wybierając preparat do antykoncepcji hormonalnej, powinno się wybrać ten o najmniejszym wpływie na metabolizm lipidów, węglowodanów oraz układ krzepnięcia. Progestageny wywierają potencjalnie niekorzystny wpływ na przemianę węglowodanową i lipidową. Wydaje się, że minitabletka gestagenna z gestagenem trzeciej generacji jest bezpieczna u kobiet chorych na cukrzycę. Nie powinno się zalecać antykoncepcji hormonalnej w cukrzycy o chwiejnym przebiegu, krótko po jej rozpoznaniu, w przypadku obecności powikłań (retinopatia, nefropatia, choroba wieńcowa serca czy miażdżyca kończyn dolnych), zwiększonego stęże-nia cholesterolu albo trójglicerydów oraz nadciśnienia tętniczego. Alternatywą dla doustnej antykoncepcji u kobiet chorych na cukrzycę może być wkładka domaciczna (intra uterine device -IUD) z zawartością miedzi (bez zawartości gestagenów). Innym istotnym problemem klinicznym, z którym często zgłaszają się kobiety chore na cukrzycę do ginekologa, są nasilone objawy klimakteryczne. Terapia hormonalna okresu menopauzy (menopausal hormone therapy -MHT) minimalizuje objawy klimakterium, ale pacjentki chore na cukrzycę często obawiają się stosowania MHT z uwagi na możliwość pogorszenia się kontroli cukrzycy. Najkorzystniejszą formą MHT w cukrzycy wydaje się stosowanie kombinacji estradiolu z octanem noretysteronu w postaci przezskórnej ze względu na brak istotnego wpływu na przemianę węglowodanową oraz zmniejszenie stężenia trójglicerydów. Preparaty przezskórne nie powodują zwiększenia stężeń trójglicerydów i ciśnienia tętniczego, co jest szczególnie ważne u chorych na cukrzycę typu 2, często skojarzoną z dyslipidemią oraz nadciśnieniem tętnicz...

Monocytes originate from progenitor cells in the bone marrow and traffic via the bloodstream to peripheral tissues. During both homeostasis and in responses to clear pathogens, circulating monocytes leave the bloodstream and migrate into tissues where they, following exposure to local growth factors, pro-inflammatory cytokines and microbial products, differentiate into macrophage or dendritic cell sub-populations. Type 1 diabetes (T1D) is a disease characterised by the elimination of the insulin producing β-cells in the pancreatic tissue through activation mainly of the adaptive immune system. In metabolic diseases, i.e. obesity, Type 2 diabetes (T2D) and diabetic complications, inflammation is mostly driven by macrophages and has been shown pivotal for disease progression even when blood glucose levels are well controlled. This review describes some current ideas and trends regarding the monocyte and macrophage involvement and their polarization in metabolic diseases that might open up novel therapeutic areas for the growing diabetic population.
