The International Classification of Headache Disorders, 2nd Edition (ICHD-II)—-Revision of Criteria for 8.2Medication-Overuse Headache

Abstract: The headache associated with medication overuse is variable and often has a peculiar pattern with characteristics shifting, even within the same day, from migraine-like to those of tension-type headache.

“…The International Classification of Headache Disorders, 2nd edition (ICHD-2) [2] and its appendix version [3] first adopted the following CDH subtypes: chronic migraine (CM), chronic tension-type headache (CTTH), new dailypersistent headache (NDPH) and hemicrania continua (HC). Many patients with CDH also have medicationoveruse headache [4,5]. Although the source of pain in primary CDH is unknown and may be dependent on the subtype, research findings suggest that the following mechanisms, alone or in combination, contribute to the process [6]: (1) abnormal excitation of peripheral nociceptive afferent fibers in the meninges; (2) enhanced responsiveness of trigeminal nucleus caudalis neurons; (3) decreased pain modulation from higher centers such as the periaqueductal gray matter; (4) spontaneous central pain generated by activation of the ''on cells'' in the medulla; (5) decreased serotonin levels; and (6) central sensitization.…”
