The Inhibitory Effect of Protamine on Platelets is Attenuated by Heparin without Inducing Thrombocytopenia in Rodents

Abstract: Protamine sulfate (PS) is a polycationic protein drug obtained from the sperm of fish, and is used to reverse the anticoagulant effect of unfractionated heparin (UFH). However, the interactions between PS, UFH, and platelets are still not clear. We measured the platelet numbers and collagen-induced aggregation, P-selectin, platelet factor 4, β-thromboglobulin, prostacyclin metabolite, D-dimers, activated partial thromboplastin time, prothrombin time, anti-factor Xa, fibrinogen, thrombus weight and megakaryocyt… Show more

“…The others showed that blockers of vasoconstrictive mediators released from platelets only partially restored normal hemodynamic and respiratory parameters in animal models and patients [ 13 , 41 , 43 , 44 ]. The partial inhibition of PS–induced hypotension by l –NAME together with the PS antiplatelet effects previously demonstrated by us indicate the involvement of short–acting mediators such as NO in the mechanism of action [ 42 ]. The delay of hypotensive effect of PS by the neutralization with UFH may also be a result of differences in the metabolism of PS or the short action of NO.…”

“…We have recently showed that both PS alone and together with UFH did not induce thrombocytopenia, platelet activation, or aggregation in rats and mice. On the contrary, PS inhibited platelet aggregation [ 42 ]. The others showed that blockers of vasoconstrictive mediators released from platelets only partially restored normal hemodynamic and respiratory parameters in animal models and patients [ 13 , 41 , 43 , 44 ].…”

“…Reoperation or reintervention is needed if bleeding happened during carotid recanalization, which is associated with the chance of perioperative stroke, MI, or even death. Miklosz et al (2019) measured the platelet numbers, collagen-induced aggregation, etc. in blood extracted from mice and rats and then furthermore found that protamine has a short-term antiplatelet activity.…”

“…PS administered alone can exhibit anticoagulant activity, inhibit thrombin and factor VII activity, impair platelet function, and negatively impact lungs and heart [ 21 , 22 ]. Those were not reported when PS was administered with UFH, which might bind PS and thus counter its effect [ 23 ].…”
