The Information–Motivation–Behavioral Skills Model of ART Adherence in a Deep South HIV+ Clinic Sample

Abstract: High levels of adherence to antiretroviral therapy (ART) are critical to the management of HIV, yet many people living with HIV do not achieve these levels. There is a substantial body of literature regarding correlates of adherence to ART, and theory-based multivariate models of ART adherence are emerging. The current study assessed the determinants of adherence behavior postulated by the Information-Motivation-Behavioral Skills model of ART adherence in a sample of 149 HIV-positive patients in Mississippi. “…IMB factors are amenable to change. [29][30][31] We have proposed several behavioral change strategies (e.g., motivational interviewing, CBT, and psychoeducation) that have potential to increase IMB components and decrease negative psychological presentations. It has previously been shown that motivational interviewing and CBT skills can successfully reduce alcohol use and improve HIV medication adherence; 50 however, no studies have assessed the efficacy of an IMB intervention that is aimed at medication adherence and methamphetamine use among an exclusively HIVpositive GBM sample.…”

“…28,30 Information refers to behavior-related information and heuristics, motivation refers to personal and social motivation, and behavioral skills, such as self-efficacy, are those that are essential to performing a specific health behavior. 31 The IMB model contends that behavior change can be achieved by increasing someone's knowledge about a particular topic (i.e., sex risk or drug use), fostering motivation to change, and developing skills needed to change behavior. 29 There is some evidence to suggest that pretreatment levels of motivation moderate response to intervention 32,33 ; however, this research has generally treated the construct of motivation in a manner that is conceptually distinct from the IMB framework.…”

“…Further, the model includes a set of moderators which may dilute the potential effect of IMB model-based interventions in some contexts (including acute psychological health issues, homelessness, and active drug abuse). The model has received support from previous research with other populations on ART and has been the basis for a number of different adherence interventions (Amico et al, 2007;Amico et al, 2009;Amico, Toro-Alfonso, & Fisher, 2005;Fisher et al, 2008). It has been previously used in risk behaviour interventions in young people (Fisher et al 1996), in investigating challenges of ART adherence in HIV-positive youth (Rongkavilit et al, 2010), and ART-related views in youth at risk of HIV infection (Simon, Altice, Moll, Shange, & Friedland, 2010).…”

“…The IMB model of ART adherence, [37] which has been used extensively in interventional adherence enhancement research [38,39] and has a developed evidence base in diverse groups of adults [39] [40] [39,41], is used as the basis for understanding specific adherence and nonadherence events. We use the situated-application of the model (sIMB) [36] to further embed the kinds of knowledge, personal and social consequences of adherence and non-adherence in the context of daily life, and skill sets needed for youth to navigate adherence in the context of self, others, and systems.…”
