The influence of margins of restorations on the periodontal tissues over 26 years

Abstract: The study confirmed the long held concept that restorations placed below the gingival margin are detrimental to gingival and periodontal health. In addition, this study suggests that the increased loss of attachment found in teeth with subgingival restorations started slowly and could be detected clinically 1 to 3 years after the fabrication and placement of the restorations. A subsequent "burn-out" effect was suggested.

“…59 This detrimental effect begins to appear 1-3 years after placement and improves concomitantly, possibly as some subgingival margins become equi-or supragingival. 60 A greater chance of gingival recession was recorded for subgingival restorations regardless of depth of sulcus penetration. 61 Thus, the role of subgingival placement of the prosthetic margins may diminish in the long-term.…”
