Parenting a child with a disability, such as neurodevelopmental disorders and genetic syndromes, implies a high level of stress. During the COVID-19 outbreak—as a period implying additional challenges—few studies have specifically investigated caregivers’ distress among neurodevelopmental disabilities. The objective of the study is to investigate whether during the COVID-19 pandemic, the level of parental stress differs between four disability groups including neurodevelopmental disorders (autism spectrum disorder (ASD), attention deficit and hyperactivity disorder (ADHD)) and genetic syndromes (Rett syndrome (RTT), Sotos syndrome (SS)) in comparison to families with typical development offspring (TD). In total, 220 Italian parents of children affected by neurodevelopmental disabilities (74 ASD, 51 ADHD, 34 SS, 21 RTT, 40 TD; age M 9.4 ± SD 4.2) underwent a standardized evaluation for stress related to parenting through the self-report questionnaire, Parental Stress Index-Short Form (PSI-SF). The main findings show greater levels of parental stress—mainly linked to child behavioral characteristics rather than parental sense of competence—in parents of children affected by a disability in comparison to children with typical development. This study highlights the need to support not only individuals with special needs but also their own caregivers: core figures in the management and outcome of children disorders.

INTRODUÇÃO: Em dezembro de 2019 surgiram, na China, os primeiros caso de COVID-19, uma doença infectocontagiosa que, em poucos meses, evoluiu para o estado de pandemia. Foram adotadas medidas de distanciamento social, que podem gerar impactos psicológicos negativos para a população. O impacto pode ser maior para os pais de indivíduos com TEA, já que as demandas de cuidados dos filhos passam para os genitores em tempo integral. Soma-se ainda o fato de o trabalho dos pais em home-office e as demandas de trabalho doméstico. OBJETIVO: Revisar estudos sobre a saúde mental e/ou qualidade de vida de pais durante a pandemia. METODOLOGIA: Trata-se de uma revisão narrativa da literatura buscando-se artigos nas bases PubMed/Medline, Scielo (Scientific Electronic Library Online), Google Scholar, Portal BVS (Biblioteca Virtual em Saúde), Periódicos Capes (Coordenação de Aperfeiçoamento de Pessoal de Nível Superior). Foram selecionados 12 artigos de estudos qualitativos transversais a partir das bases de dados. RESULTADOS: A avaliação dos artigos mostra que os pais de indivíduos com TEA tendem a estar mais ansiosos, estressados e depressivos nesse período devido a sobrecargas físicas e emocionais. Além disso, eles apresentaram pior qualidade de vida neste período quando comparado a pais de crianças com outros transtornos e com desenvolvimento típico. CONCLUSÃO: O desenvolvimento de mais estudos sobre o tema é necessário, pois com maior embasamento científico é possível planejar um cuidado mais direcionado. O apoio virtual de profissionais e outros pais podem minimizar o impacto da pandemia sobre essa população. Entretanto, a diversidade de fatores sociais, econômicos e culturais envolvidos nessa questão requer mobilização multisetorial. Palavras-chave: Transtorno do espectro autista. Pais. Saúde Mental. Background: Since COVID-19 has become pandemic, everyday life has seen dramatic changes affecting individuals, families, and children with and without autism. These entail, among other things, more time at home, digital forms of communication, school closures, and reduced support and intervention. Aim: Here we aim to systematically assess the effects of the pandemic on autistic and neurotypical (NT) children, and pursue a pre-registered hypothesis suggested by initial observations that some children with autism might respond comparatively well to a situation of constrained, predictable, and digital social communication during the pandemic. Methods: We provide a systematic review of current relevant work, including 76 papers selected from a database search until February 1, 2021. We then report survey data from several countries assessing activities, well-being (PedsQLTM), and social life in families with autism, and their respective change at various stages of the pandemic. We focus on differences between children with and without autism from within the same families, and on different outcomes for children with high- or low-functioning autism. Results: While children with autism scored lower in emotional and social functioning than their NT siblings, comparable decreases in well-being and increases in anxiety were seen for both groups, compared to before the pandemic. By contrast, decreases in adaptability were significantly more pronounced in autistic than NT children. Preliminary results suggest that changes were comparable for children with high- and low-functional autism overall, but individual differences prevail. Conclusions: Overall, although individual families do report some positive effects of pandemic restrictions, our data give no clear evidence that these generalize across children with autism, or even just to children with high-functioning autism. We discuss significant challenges for children with and without autism that need to be addressed to protect childrens’ well-being under pandemic conditions but also point to potential in the present situation that could be used towards social participation and success in older children and young adults with autism.
