The influence of a doppler ultrasound in arteriovenous fistula for dialysis failure related to some risk factors

Abstract: Introduction: The increasing prevalence of chronic kidney disease has increased the demand for arteriovenous fistula (AVF) care. The objective of this study was to assess the relationship between some risk factors for AVF failure (advanced age, female sex, diabetes, obesity, central venous catheter, previous fistula, and hospitalization) and having a Doppler ultrasound performed preoperatively. Methods: A prospective study was performed with 228 dialysis patients from Imperatriz, Maranhão. Half of the sample … Show more

“…The increase in the creation of AVFs has shown a rate of primary of maturation failures from 23% to 46%, with a great need for interventions to reach maturity. [1][2][3][4][5] The use of catheters as a bridge for a new AVF has become frequent, and AVGs were reserved for patients without the possibility of having an AVF. Knowledge in vascular access has advanced, and the evidence challenges the dogma that an AVF is always the best access for all patients.…”

“…The authors commented on the lack of infrastructure for HD vascular access, especially surgical and endovascular suites, and how the lack of endovascular treatments undermines the creation of vascular accesses, as up to 46% would need an intervention to mature. The impossibility of treating primary failures may lead to the creation of a new AV access, reducing the available creation sites and leading to longer catheter exposure and potential complications (24)(25)(26)(27)(28). Brazil has no regulations for outpatient endovascular procedures; therefore, patients with dysfunctional access need to be hospitalized for treatment.…”

“…Several factors such as vessel diameter and quality, surgeon experience, and hemodynamic conditions have been associated with successful AVF maturation 9 . Doppler ultrasound to map and decide which vessels to use in creating an AVF [10][11][12] has been suggested in patients at higher risk of maturation failure 4 . However, besides vascular parameters 4,12 , other clinical characteristics have not been consistently associated with successful AVF maturation in predictive models 8,13 .…”
