The incidence of complications associated with retrobulbar injection of anaesthetic for ophthalmic surgery

Abstract: A retrospective study of the incidence of complications associated with the use of retrobulbar injections was carried out on a sample of 1083 consecutive patients. The overall incidence of retrobulbar haemorrhage (1.3%) compared favourably with previous reports, but was found to be related to the experience of the surgeon. None of the more serious ocular complications of retrobulbar injection occurred in this sample. The relative safety of this technique is discussed.

“…The risk for a retrobulbar hemorrhage after retrobulbar injection is much higher than that for a perforation; the reported hemorrhage rate is between 0.4% and 1.7%. [19][20][21] When sharp needles are used, the risk can increase to 5.0%. 22 Using a cannula longer than 40.0 mm can cause a direct lesion of the optic nerve, as Unsö ld et al 3 found using computed tomography reconstructions.…”

“…Jedná se o tradiční subspecializaci oftalmologie, jejíž kořeny sahají až do starověku, první literární zmínku ale nalézáme až roku 1925 v učebnici E. Spaetha [1]. V roce 1969 byla tato subspecializace konečně formálně definována vznikem Americké společnosti oční plas-tické a rekonstrukční chirurgie [2]. Péče o oční traumata je pak nedílnou a důležitou součástí této odbornosti.…”

“…[32][33][34][35] The risk of vision-threatening hemorrhage in association with peribulbar and retrobulbar anesthesia has been examined in multiple studies and more contemporary data suggest that this is rare, even with continued antithrombotic treatment.
