The implications of the presence of an aberrant right hepatic artery in patients undergoing a pancreaticoduodenectomy

Abstract: The surgical and oncological outcomes of PD remain unaffected by the presence of ARHA provided that the anatomy is recognized and appropriately managed. Aberrant right hepatic artery can be classified into three types according to their anatomical relationship with the head of the pancreas.

“…Classically, the presence of an arterial variant could be a contraindication for surgery (12). Nowadays, we know that oncologic surgery of the head of the pancreas is feasible in the presence of a variant hepatic artery (5). Arterial variant involves a technical difficulty that may determine a more conservative resection of the pancreas, especially for retropancreatic plane and lead an inadequate removal of the tumor (9).…”

“…Probably the presence of a RHA influenced in making a more radical and meticulous resection of the retropancreatic margin. According to Jah et al (5) variant of the right hepatic artery may have three different courses in relation to the head of the pancreas, which may have important consequences for pancreaticoduodenectomy (5).…”

“…Final results have been submitted on a randomized multicenter trial that investigated surgery versus radiochemotherapy in the treatment of resectable locally invasive pancreatic cancer. 21 Surgical techniques are the topics of many articles, including a multicenter study showing that extrahepatic bile duct resection did not contribute to better prognosis in advanced gallbladder carcinoma, 22 along with reports on the signifi cance of aberrant right hepatic artery 23 and of pylorus preservation 24 in pancreaticoduodenectomy. In addition to the above-mentioned clinical reports that were submitted, basic research fi ndings have been reported, such as the effect of FTY720 in small bowel transplantation 25 and of endothelial molecular markers in the evaluation of acute pancreatitis severity.…”
