Purpose of review Vitamin deficiency is a risk factor in the development of peripheral neuropathy, which leads to complex and severe diseases. This review provides an update overview of the literature on the roles of vitamins in peripheral neuropathy, highlighting their pathophysiological and therapeutic roles. Recent findings The importance and clinical manifestations and implications of the vitamins and vitamin deficiencies are further demonstrated in peripheral neuropathy and the associated diseases. Vitamin deficiency is common in various severe and complex diseases such as diabetes, chemotherapy, acute nutritional axonal neuropathy, dermatitis, complex regional pain syndrome, postherpetic neuralgia, carpal tunnel syndrome, and so forth and some rare clinical case reports. There is evidence that deficiencies of almost all vitamins are associated with diabetic neuropathy. Vitamin supplementation may serve as an effective therapeutic strategy. Summary The vitamins play critical roles in maintaining physiological functions, and vitamin deficiencies cause peripheral neuropathy with various severe and complex diseases. The therapeutic benefits of vitamins and further understanding of the mechanisms for vitamin treatment effects should be emphasized and highlighted. More clinical trials are needed to establish optimal treatment strategies for vitamins in the various neuropathies. A large range of people/patients screening for vitamin deficiencies may be considered in order to provide early diagnosis and timely medical assistance.

Propósito: la neuropatía periférica tiene un espectro clínico inespecífico y multifactorial, con frecuente subdiagnóstico y terapéutica de eficacia variable. Existe una heterogénea prescripción de vitaminas B, las cuales pueden desempeñar un rol importante en el manejo de diferentes neuropatías; sin embargo, en Colombia no existen guías clínicas al respecto. El propósito de este trabajo es orientar en el reconocimiento temprano de las neuropatías periféricas y genera recomendaciones sobre el uso adecuado de vitaminas B neurotrópicas. Descripción de la metodología: acuerdo de expertos sobre la neuropatía periférica y el rol terapéutico de las vitaminas B con énfasis en la epidemiología en Colombia, diagnóstico y tratamiento. Contenidos: en Colombia, la prevalencia de neuropatía periférica se estima en alrededor del 10?%, sin embargo, no hay datos recientes. Dentro de las etiologías más frecuentes se encuentran la neuropatía diabética, infecciosas, inflamatorias, carenciales, toxicas y farmacológicas. Se recomiendan las siguientes herramientas de tamizaje en población de riesgo: DN4, MNSI, test de monofilamento, test de vibración y valoración de reflejos. Las vitaminas B1, B6 y B12 son seguras, accesibles y pueden ser eficaces en neuropatía periférica, incluso cuando el déficit no ha sido demostrado, pero con requerimientos particulares en su administración conjunta. Conclusiones: las neuropatías periféricas son un reto diagnóstico y terapéutico que requiere la identificación oportuna para el tratamiento de la etiología subyacente y el control de síntomas. El uso de vitaminas B neurotrópicas es efectivo y seguro en neuropatía periférica carencial, y también parece ser eficaz en el manejo de neuropatías periféricas de diferentes etiologías.

B12 deficiency has a higher prevalence in type 2 diabetes mellitus (T2DM) patients. The causes of this observation are poorly understood. Self-imposed (vegan/vegetarian) or advised dietary modification or intake of metformin could be contributing. Hence, we undertook this study to evaluate the dietary intake of Vitamin B12 in diabetes patients in comparison with age-matched healthy controls. Fifty T2DM patients (cases) and 50 age-matched volunteers (controls) aged 35-60 years were enrolled at a tertiary hospital, in New Delhi. Sociodemographic and dietary information was gathered and the average dietary intake of B12 was estimated using the Cobalamin Intake in North Indians-Food Frequency Questionnaire (COIN-FFQ) (developed and validated). Medical records were reviewed for biochemical parameters (HbA1c, lipid profile, and blood sugar) of the last three months. The mean age was 50.66±6.09 years (n=100) and the mean body mass index of 28.49±4.53 kg/m2. The mean intake of dietary B12 was 3.2±1.7 μg/day and 3.8±1.4 μg/day (p=0.047) in diabetes patients and controls respectively. Thirty-six percent of diabetes patients did not meet the currently recommended daily dietary intake of B12 intake (vs. 14% of controls; p=0.011). Diabetes patients have a lower dietary intake of Vitamin B12 attributable to their distinctive dietary pattern. It is necessary to explore the factors leading to lower dietary B12 intake in diabetes patients.
