The Impact of Upper Face Botulinum Toxin Injections on Eyebrow Height and Forehead Lines: A Randomized Controlled Trial and an Algorithmic Approach to Forehead Injection

Abstract: No previous study has objectively evaluated the effect of different forehead injection patterns on the eyebrow height and forehead lines. The patients were divided into three groups. Botulinum toxin was injected into both the lateral and medial eyebrow depressors in all groups. The frontalis was injected using either a V-pattern (group 1), a middle horizontal pattern (group 2), or a high horizontal pattern (group 3). Objective eyebrow measurements were performed using standardized preinjection and postinjectio… Show more

“…However, in their results, the upper horizontal pattern, which was injected 1 cm caudal to the hair line, exhibited inferior results to the other two injection patterns. 15 This high level of injection used in their study probably did not coincide with the C-line; hence, the injected toxin was not able to adequately block the action of the frontalis, which could provide an explanation for such inferior results.…”

“…Although the levator is stimulated by Botox injection into the frontalis, this may not suffice to increase the palpebral height, due to the eyebrow depression effect. 12 Thread insertion exerts both fixation and tightening effects. 3 Therefore, in contrast to Botox, the eyebrow is also elevated.…”

“…We empirically assumed that the increased levator function resulting from frontalis paralysis improves eyelid ptosis, but this possibility has not been proven yet. Although the levator is stimulated by Botox injection into the frontalis, this may not suffice to increase the palpebral height, due to the eyebrow depression effect 12 …”

“…Theoretically, targeting of the brow depressors, which includes the orbicularis oculi, depressor supercilia, and corrugator supercilia, facilitates unopposed brow elevation by the frontalis (Sundaram & Kiripolsky, 2013). In the elderly, it is usually injected into the eyebrow depressor and forehead simultaneously, and in such cases, it is important to target the upper part of the forehead, rather than the lower and the mid-forehead, in order to achieve a significant brow lift (Jabbour et al, 2018).…”
