The New Rural Cooperative Medical System (NRCMS) is one of the essential systems for ensuring public health in rural China. This paper investigates the effect of farmers' participation in the NRCMS on their subjective well-being and its mechanisms using data from the Chinese General Social Survey 2017. The results show that farmers' participation in the NRCMS significantly enhances their subjective well-being, and these results remain robust after regression with the instrumental variables method and propensity score matching method. Further analysis of the mechanisms suggests that participation in the NRCMS can enhance farmers' subjective well-being by increasing their consumption levels other than medical consumption. Moreover, medical consumption levels play a negative role in participating in the NRCMS on farmers' subjective well-being, which can be explained as the “masking effect.” The regression results of the subsamples show that the higher a farmer's income is, the less his or her participation in the NRCMS enhances subjective well-being. And the effect of participation in the NRCMS on farmers' subjective well-being is not significant if their health status is too high or too low.

Background The widespread outbreak of the COVID-19 pandemic poses a significant threat to human health. In the context of Healthy China, enhancing residents' health performance is an inevitable choice to promote the equalization of public medical and health services. Methods This article utilized online survey data, employs the O-probit model and Health Equity Gap Model to empirically analyze the relationship between COVID-19 infection, the medical insurance system, and health equity. Results Findings revealed that as of mid-January 2023, the nationwide infection rate of COVID-19 in China is 82.34%, with 24.86% of respondents reporting the loss of relatives or friends, and the average age of death being 71.30 years old. In terms of impact of COVID-19, Chinese residents are most concerned about the future impact of COVID-19 on their health and work. Compared to the Urban-Rural Resident Medical Insurance in China, the Urban Employee Medical Insurance is more effective in alleviating the negative impact of COVID-19 infection on residents' health, especially for middle-aged and elderly individuals, rural residents, those with chronic diseases, and in the eastern regions of China. Urban Employee Medical Insurance improves residents' health by influencing their drug purchase amount and outpatient visits following the impact of COVID-19. If Chinese residents receive the same treatment as Urban Employee Medical Insurance, their health would improve by 0.240 units, equivalent to 56.21% of the actual health gap between different medical insurance coverage groups. Conclusions Compared to the Urban-Rural Resident Medical Insurance, the Urban Employee Medical Insurance is more effective in mitigating the negative impact of COVID-19 infection on residents' health. Under the impact of the COVID-19 pandemic, different medical insurance systems have widened the health gap between urban employees and rural residents.

IntroductionEnhancing the wellbeing of residents through universal health coverage (UHC) is a long-term policy goal for China. In 2016, China integrated the New Rural Cooperative Medical Scheme (NRCMS) and the Urban Resident Basic Medical Insurance (URBMI) into the Urban and Rural Resident Basic Medical Insurance (URRBMI) to address the problem of fragmentation.ObjectiveThe objective of this study was to investigate whether the integration of basic medical insurance had an impact on the subjective wellbeing of Chinese residents.MethodsUsing the China Household Finance Survey data of 2015 and 2019, we empirically estimated the influence of the integration of basic medical insurance on Chinese residents through the difference-in-difference method based on propensity score matching (PSM-DID).ResultsOur findings indicate that the integration of basic medical insurance improved the subjective wellbeing of the insured population. Additionally, through heterogeneity testing, we validated that the integration increased the subjective wellbeing of residents from less developed regions in West China and rural areas, as well as those with older adult dependents. However, the subjective wellbeing of low-income groups, who were expected to benefit more from the URRBMI, did not improve significantly, at least in the short term.ConclusionAccording to our research, the integration of basic medical insurance in China supports the country's objective of achieving equality and providing universal benefits for its residents. The introduction of the URRBMI has had a positive impact on the subjective wellbeing of insured individuals. This is particularly beneficial for disadvantaged groups in less developed regions, as well as for residents with older adult dependents. However, the subjective wellbeing of the middle-income group has improved significantly, whereas that of the low-income group, despite being the intended beneficiaries of the integration, did not show significant improvement.RecommendationsFrom a funding perspective, we recommend establishing a dynamic adjustment funding system that links residents' medical insurance funding standards with their disposable income. Regarding the utilization of the URRBMI, the benefit packages should be expanded, particularly by covering more outpatient services through risk pooling. We call for further research with additional data and continued efforts on improving wellbeing of residents, particularly for disadvantaged populations.
